经颅磁刺激联合综合康复训练治疗小儿脑瘫的效果及对认知功能的影响

Efficacy of transcranial magnetic stimulation combined with comprehensive rehabilitation training in children with cerebral palsy and its influence on cognitive function

摘要 目的探讨经颅磁刺激联合综合康复训练治疗小儿脑瘫的效果及对认知功能的影响。方法收集2021年6月至2023年6月就诊于泉州市妇幼保健院·儿童医院的60例脑瘫患儿。按照随机数字表法分为对照组与观察组,每组30例。对照组予以综合康复训练治疗,观察组在对照组的基础上予以经颅磁刺激治疗。采用肌电图/诱发电位仪对中枢传导时间进行检测,并将体感诱发电位进行分级。使用脑瘫儿童精细运动能力评估量表(FMFM)评估运动功能,使用Gesell发育诊断量表(GDS)评估患儿认知功能。结果治疗后,观察组体感诱发电位分级比较,差异有统计学意义(P<0.05)。治疗后,两组患儿FMFM和GDS评分均升高,且观察组高于对照组,差异均有统计学意义(P<0.05)。结论小儿脑瘫患者采用综合康复训练联合经颅磁刺激治疗,有助于促进脑神经恢复,改善患儿认知功能和运动功能。 Objective To explore the efficacy of transcranial magnetic stimulation combined with comprehensive rehabilitation training in children with cerebral palsy and its influence on cognitive function.Methods A total of 60 children with cerebral palsy admitted to Quanzhou Maternal and Child Health Hospital from June 2021 to June 2023 were divided into control group and observation group according to random number table method,30 cases in each group.The children in the control group were treated with comprehensive rehabilitation training,and children in the observation group were given transcranial magnetic stimulation on the basis of the control group.The central conduction time was measured by electromyography/evoked potentiometer,and somatosensory evoked potentials were graded.Motor function was assessed using the fine motor function measure(FMFM)for children with cerebral palsy,and cognitive function was assessed using the Gesell developmental diagnostic scale(GDS).Results After the treatment,there was statistical significance in the classification of somatosensory evoked potential between the two groups(P<0.05).After the treatment,FMFM and GDS scores in both groups were increased,and more higher in the observation group,with statistical significance(P<0.05).Conclusion Comprehensive rehabilitation training combined with transcranial magnetic stimulation for children with cerebral palsy is helpful to promote cerebral nerve recovery and improve cognitive and motor function.
