LDH、NT-proBNP、ALB水平对不完全性川崎病的早期诊断价值

Early Diagnostic Value of LDH,NT-proBNP and ALB Levels in Incomplete Kawasaki Disease

摘要 目的:探讨乳酸脱氢酶(LDH)、N末端脑钠肽前体(NT-proBNP)、白蛋白(ALB)水平对不完全性川崎病(IKD)的早期诊断价值。方法:选取巴彦淖尔市医院2020年1月—2023年1月收治的80例IKD患儿作为IKD组,选取本院同期收治的80例典型川崎病(KD)患儿作为KD组。所有患儿入院后检测LDH、NT-proBNP、ALB水平,对比IKD组与KD组一般情况和LDH、NT-proBNP、ALB水平。通过受试者操作特征(ROC)曲线分析LDH、NT-proBNP、ALB水平对IKD的早期诊断价值。同时将80例IKD患儿依照冠脉损伤情况分为两个亚组,即无损伤组(n=30)和损伤组(n=50),对比两组LDH、NT-proBNP、ALB水平。结果:IKD组和KD组性别、年龄及高热、多形性皮疹、口腔黏膜变化占比对比,差异均无统计学意义(P>0.05);IKD组发热时间长于KD组,指端改变、结膜充血、颈部淋巴结肿大占比低于KD组,差异均有统计学意义(P<0.05);IKD组LDH、NT-proBNP、ALB水平均明显高于KD组,差异均有统计学意义(P<0.05);LDH、NT-proBNP、ALB三者联合对IKD的诊断效能优于单一检测;损伤组LDH、NT-proBNP、ALB水平均明显高于无损伤组,差异均有统计学意义(P<0.05)。结论:LDH、NT-proBNP、ALB水平对IKD的早期诊断价值较高,且临床可考虑通过LDH、NT-proBNP、ALB三者联合来诊断IKD。 Objective:To explore the early diagnostic value of lactate dehydrogenase(LDH),N-terminal pro-brain natriuretic peptide(NT-proBNP),albumin(ALB)levels in incomplete Kawasaki disease(IKD).Method:A total of 80 children with IKD treated in Bayannur Hospital from January 2020 to January 2023 were selected as IKD group,and 80 children with typical Kawasaki disease(KD)treated in our hospital during the same period were selected as KD group.LDH,NT-proBNP and ALB levels of all children were detected after admission,and the general situation and LDH,NT-proBNP and ALB levels of IKD group and KD group were compared.The early diagnostic value of LDH,NT-proBNP and ALB levels in IKD were analyzed by receiver operator characteristic(ROC)curve.At the same time,80 children with IKD were divided into two subgroups according to the status of coronary artery injury,namely the non-injury group(n=30)and the injury group(n=50),and the levels of LDH,NT-proBNP and ALB were compared between the two groups.Result:There were no significant differences in gender,age and the proportion of high fever,pleomorphic exanthem and oral mucosal changes between IKD group and KD group(P>0.05).The fever time in IKD group was longer than that in KD group,and the proportion of finger tip changes,conjunctival congestion and cervical lymph node tumefaction were lower than those in KD group,the differences were statistically significant(P<0.05).The levels of LDH,NT-proBNP and ALB in IKD group were significantly higher than those in KD group,the differences were statistically significant(P<0.05).The diagnostic efficiency of LDH,NT-proBNP and ALB in combination was better than that of single detection,the levels of LDH,NT-proBNP and ALB in the injury group were significantly higher than those in the non-injury group,the differences were statistically significant(P<0.05).Conclusion:LDH,NT-proBNP and ALB levels have a high value in the early diagnosis of IKD,and the combination of LDH,NT-proBNP and ALB can be considered to diagnose IKD.
