摘要
目的探究腹腔镜疝囊高位结扎术治疗小儿腹股沟斜疝临床效果。方法选取2020年4月至2023年8月平顶山市妇幼保健院收入65例小儿腹股沟斜疝患儿,经随机数字表法分为两组,对照组33例予以小切口疝囊高位结扎术,观察组32例予以腹腔镜疝囊高位结扎术,对比两组手术相关指标、C反应蛋白水平、疼痛评分及不良反应率、复发率。结果观察组术中出血量少于对照组,手术时间、术后首次下床活动所用时间、切口愈合时间以及住院时间均短于对照组,差异均有统计学意义(P<0.05);术后12 h、48 h的C反应蛋白水平低于对照组,差异均有统计学意义(P<0.05);术后12 h、术后24 h、术后48 h疼痛评分均低于对照组,差异均有统计学意义(P<0.05);观察组不良反应发生率(3.12%)、复发率(0),低于对照组(21.21%、15.15%),差异均有统计学意义(P<0.05)。结论腹腔镜疝囊高位结扎术在小儿腹股沟斜疝治疗中能获得满意成效,缩短患者病程康复时间,且减轻术后炎症指标及疼痛感,且术后不良反应率、复发率偏低,值得应用。
Objective To explore the clinical effect of laparoscopic high ligation of the hernia sac in the treatment of pediatric inguinal hernia.Methods A total of 65 children with inguinal hernia admitted to Pingdingshan Maternal and Child Health Hospital from April 2020 to August 2023 were selected and randomly divided into two groups using a random number table method.The control group(33 cases)underwent small incision high ligation of the hernia sac,while the observation group(32 cases)underwent laparoscopic high ligation of the hernia sac.Surgical-related indicators,C-reactive protein levels,pain scores,adverse reaction rates,and recurrence rates were compared between the two groups.Results The observation group had less intraoperative bleeding,shorter operation time,shorter time to first postoperative ambulation,shorter incision healing time,and shorter hospital stay compared to the control group,with statistically significant differences(P<0.05).The C-reactive protein levels at 12 hours and 48 hours postoperatively were lower in the observation group than in the control group,with statistically significant differences(P<0.05).Pain scores at 12 hours,24 hours,and 48 hours postoperatively were lower in the observation group than in the control group,with statistically significant differences(P<0.05).The observation group had lower rates of adverse reactions(3.12%)and recurrence(0)compared to the control group(21.21%,15.15%),with statistically significant differences(P<0.05).Conclusion Laparoscopic high ligation of the hernia sac achieves satisfactory results in the treatment of pediatric inguinal hernia,shortens the recovery time,reduces postoperative inflammatory markers and pain,and has lower rates of postoperative adverse reactions and recurrence.It is worth applying.
