摘要
目的研究益气清毒化瘀汤加减联合奥沙利铂+卡培他滨(XELOX)方案治疗老年结肠癌的效果及对炎症因子、免疫功能的影响。方法选取2021年12月—2023年1月在东台市人民医院进行治疗的老年结肠癌患者80例,随机分为观察组和对照组各40例,对照组采用XELOX方案,观察组采用益气清毒化瘀汤加减联合XELOX方案治疗,比较两组患者的中医证候积分、治疗效果、炎症因子水平[C反应蛋白、白细胞介素-4(interleukin-4,IL-4)、白细胞介素-6(interleukin-6,IL-6)]、免疫功能(CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+))、不良反应发生率。结果(1)治疗前,两组患者中医证候积分接近(P>0.05);治疗后,两组患者中医证候积分均降低,且观察组较对照组而言更低(P<0.05);(2)治疗后,观察组患者治疗效果好于对照组(P<0.05);(3)治疗前,两组患者C反应蛋白、IL-4、IL-6水平比较差异无统计学意义(P>0.05);治疗后,两组患者上述水平均降低,且观察组较对照组而言更低(P<0.05);(4)治疗前,两组患者CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+)水平比较差异无统计学意义(P>0.05);治疗后,两组患者CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平均升高,且观察组高于对照组(P<0.05),两组患者CD_(8)^(+)水平均降低,且观察组较对照组而言更低(P<0.05);(5)治疗后,两组患者不良反应发生率接近(P>0.05)。结论采用益气清毒化瘀汤加减联合XELOX方案治疗老年结肠癌患者可提高其中医证候积分,并对患者炎症因子水平、免疫功能均起到改善作用,效果显著,安全性高。
Objective To study the effect of Modified Yiqi Qingdu Huayu Decoction(益气清毒化瘀汤)combined with oxaliplatin+capecitabine(XELOX)in the treatment of elderly colon cancer and the influence on inflammatory factors and immune function.Methods Eighty elderly patients with colon cancer who received treatment in the hospital from December 2021 to January 2023 were selected as the study subjects and were randomly divided into observation group and control group with 40 cases in each group.The control group was treated with XELOX regimen,and the observation group was given Modified Yiqi Qingdu Huayu Decoction combined with XELOX regimen.TCM syndromes scores,therapeutic effect,the levels of inflammatory factors[C-reactive protein,interleukin-4(IL-4),interleukin-6(IL-6)],immune function(CD_(4)^(+),CD_(8)^(+),CD_(4)^(+)/CD_(8)^(+))and incidence rates of adverse reactions were compared between the two groups of patients.Results(1)Before treatment,the TCM syndromes scores in the two groups had no significance(P>0.05).After treatment,the scores of TCM syndromes in both groups were reduced,and the scores in the observation group were lower compared with those in the control group(P<0.05).(2)After treatment,the therapeutic effect in the observation group was better than that in the control group(P<0.05).(3)There were no significant differences in the levels of C-reactive protein,IL-4 or IL-6 between both groups before treatment(P>0.05).After treatment,the above levels were decreased in both groups,and the levels in the observation group were lower compared to those in the control group(P<0.05).(4)Before treatment,there were no significant differences in the levels of CD_(4)^(+),CD_(8)^(+)or CD_(4)^(+)/CD_(8)^(+)between the two groups(P>0.05).After treatment,the levels of CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in both groups were enhanced,and the levels were higher in the observation group than those in the control group(P<0.05),and the level of CD_(8)^(+)in the two groups was reduced,and the level in observation group was lower than that in control group(P<0.05).(5)After treatment,the incidence rates of adverse reactions had no significance in the two groups(P>0.05).Conclusion The combination of Modified Yiqi Qingdu Huayu Decoction and XELOX regimen in the treatment of elderly patients with colon cancer can enhance the TCM syndromes scores and improve the inflammatory factors and immune function of patients,with significant effect and high safety.
