儿童肺炎支原体肺炎共感染病原谱分析

摘要 目的:了解儿童肺炎支原体肺炎(Mycoplasma pneumoniae pneumonia,MPP)共感染的病原谱情况,为临床及时正确诊断、合理有效治疗提供依据及指导。方法:回顾性分析2023年01月01日至2023年12月31日期间在上饶市人民医院儿科住院的29 d~16 w岁MPP患儿共感染的病原谱,及MPP共感染病原谱在不同年龄段、不同性别、不同季节的差异。结果:291例MPP患儿中,与病毒共感染209例(18.79%),其中婴幼儿组与呼吸道合胞病毒共感染检出率高于另两组(P<0.05);乙型流感病毒高于学龄前组(P<0.05);学龄组腺病毒检出率高于婴幼儿组(P<0.05);学龄组、学龄前组甲型流感病毒共感染检出率高于婴幼儿组(P<0.05)。291例患儿中与细菌共感染共检出90例(8.09%),不同年龄组患儿细菌检出率则无种属类型差异(P>0.05),前三分别为肺炎链球菌、金黄色葡萄球菌、流感嗜血杆菌;1112例MPP患儿中,发生共感染的男性占比27.74%,女性占比24.19%,不同性别之间差异无统计学意义(P>0.05);MPP共感染一年四季均可发病,以冬季发病率最高,差异有显著统计学意义(P<0.05)。结论:不同年龄段MPP患儿与病毒共感染情况存在一定差异,MPP共感染一年四季均可发病,以冬季发病率最高。因此本地区临床应重视不同季节及不同年龄段患儿的MPP共感染情况。 Objective:To understand the pathogen spectrum of co-infection in children with Mycoplasma pneumoniae pneumonia(MPP),so as to provide a basis and guidance for timely and correct diagnosis,reasonable and effective treatment in clinical practice.Methods:A retrospective analysis was performed on the pathogen spectrum of co-infection in children with MPP who were between 29 days and 16 weeks old and hospitalized in Pediatrics Department of Shangrao People's Hospital from January 1 to December 31,2023.Meanwhile,the differences in the pathogen spectrum of co-infection in children with MPP in different age groups,of different genders and in different seasons were analyzed.Results:Among 291 children with MPP,there were 209(18.79%)children with co-infection.The detection rate of co-infection with respiratory syncytial virus in the infant group was higher than those in the school-age group and the preschool group(P<0.05).The detection rate of influenza B virus was higher than that in the preschool group(P<0.05).The detection rate of adenovirus in the school-age group was higher than that in the infant group(P<0.05).The detection rates of co-infection with influenza A virus in the school-age group and the preschool group were higher than that in the infant group(P<0.05).Among the 291 children,90(8.09%)children with co-infection with bacteria were detected.The bacterial detection rates in different age groups showed no difference in terms of species(P>0.05).The top three were Streptococcus pneumoniae,Staphylococcus aureus,and Haemophilus influenzae.Among the 1112 children with MPP,males and females with co-infection accounted for 27.74%and 24.19%,indicating no statistically significant difference between different genders(P>0.05).Co-infection of MPP occurred all the year round,and the incidence rate was the highest in winter(P<0.05).Conclusion:Co-infection in children with MPP and co-infection of different age is different.Co-infection in MPP occurs all the year round,and the incidence rate is the highest in winter.Therefore,attention should be paid to co-infection of MPP in different seasons and in children of different age in this area.
