摘要
目的研究加巴喷丁联合依帕司他治疗糖尿病痛性神经病变(PDPN)的效果及对患者神经传导速度的影响。方法队列研究。抽取2020年1月至2022年12月新乡医学院第三附属医院收治的PDPN患者98例,按随机数字表法分为单一组与联合组,每组49例。单一组采用依帕司他治疗,联合组在单一组基础上联用加巴喷丁。比较两组临床疗效、神经病变症状评分(TTSS)、正中神经和腓神经神经传导速度[感觉传导速度(SCV)和运动传导速度(MCV)]及不良反应发生率。结果联合组总有效率(93.88%,46/49)高于单一组(71.43%,35/49),P<0.05。治疗后,联合组疼痛、烧灼感、感觉异常、麻木评分低于单一组(P<0.05);治疗后,联合组正中神经和腓神经SCV、MCV高于单一组(P<0.05)。联合组不良反应发生率(12.24%,6/49)与单一组(8.16%,4/49)比较差异未见统计学意义(P>0.05)。结论加巴喷丁联合依帕司他治疗PDPN效果显著,能缓解患者神经病变症状,增加正中神经和腓神经神经传导速度,且无明显不良反应。
Objective To study the effect of gabapentin combined with epalrestat on painful diabetic peripheral neuropathy(PDPN)and its influence on nerve conduction velocity.Methods A total of 98 patients with PDPN treated in the Third Affiliated Hospital of Xinxiang Medical University from January 2020 to December 2022 were selected for the cohort study.And they were divided into a single group and a combined group using a random number table method,with 49 cases in each group.The single group received epalrestat treatment,and the combined group received gabapentin based on the treatment of the single group.The efficacy,total symptoms score(TTSS),conduction velocity of median nerve and fibular nerve assessed by sensory conduction velocity(SCV)and motor conduction velocity(MCV),and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the combined group(93.88%,46/49)was higher than that of the single group(71.43%,35/49),P<0.05.Compared with the single group,the pain,burning sensation,paresthesia,and numb scores of the combined group were lower after treatment(P<0.05).Compared with the single group,the SCV and MCV of median nerve and fibular nerve in the combined group were higher after treatment(P<0.05).There was no significant difference in the incidence of adverse reactions between the combined group(12.24%,6/49)and the single group(8.16%,4/49),P>0.05.Conclusions Gabapentin combined with epalrestat is an effective method of the treatment for PDPN,which can relieve the symptoms of nerve damage,and increase conduction velocity of the median nerve and fibular nerve,without significant adverse reactions.
