摘要
目的:探讨左甲状腺素钠片联合加味补中益气汤治疗原发性甲状腺功能减退症的临床效果。方法:选取2022年10月—2023年10月黔东南州中医医院收治的74例原发性甲状腺功能减退症患者为研究对象,按照每组37例将74例研究对象随机分为观察组(治疗方案:左甲状腺素钠片25μg/次,1次/d;加味补中益气汤2次/d)、对照组(治疗方案:左甲状腺素钠片25μg/次,1次/d)。对比两组甲状腺激水平、生活质量、中医证候积分及不良反应发生率。结果:治疗后,两组促甲状腺激素水平降低,游离三碘甲状腺原氨酸、血清游离甲状腺激素水平升高,观察组促甲状腺激素水平低于对照组,游离三碘甲状腺原氨酸、血清游离甲状腺激素水平高于对照组(P<0.05);治疗后,两组睡眠质量、心理情绪、自我照顾能力、疼痛状况评分升高,观察组高于对照组(P<0.05);治疗后,两组中医证候评分降低,观察组的评分比对照组更低(P<0.05);两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论:对于原发性甲状腺功能减退患者,在左甲状腺素钠片基础上应用加味补中益气汤可显著改善其甲状腺功能,使得甲状腺激素水平趋于正常范围,还可改善临床症状,提高生活质量,且安全性较高。
Objective:To investigate the clinical effect of Jiawei Buzhong Yiqi Decoction on primary hypothyroidism.Methods:74 patients with primary hypothyroidism admitted to Qiandongnan Hospital of Traditional Chinese Medicine from October 2022 to October 2023 were selected into the study group including 77 patients in each group(treatment regimen:levothyroxine sodium tablets 25μg each time,once a day;Jiawei Buzhong Yiqi Decoction twice a day)and the control group(treatment plan:levothyroxine sodium tablets 25μg each time,once a day).The levels of thyroid hormones,traditional Chinese medicine syndrome scores,quality of life,and incidence of adverse reactions between two groups were compared.Results:After treatment,thyroid-stimulating hormone level decreased,free triiodothyronine and free thyroxine levels increased,thyroid-stimulating hormone level in observation group was lower than control group,free triiodothyronine and free thyroxine levels were higher than control group(P<0.05);after treatment,the scores of sleep quality,psychological emotion,self-care ability and pain status of two groups were increased,and the observation group was higher than the control group(P<0.05);after treatment,the TCM syndrome score of the two groups decreased,and the observation group was lower than the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:For patients with primary hypothyroidism,the application of Jiawei Buzhong Yiqi Decoction on the basis of levothyroxine tablets can significantly improve their thyroid function,make the level of thyroid hormones converge to the normal range,but also improve the clinical symptoms,improve the quality of life,and have a high degree of safety.
