摘要
目的 探究直接数字化X射线摄影(DR)检查对肺部小结节的初步判断。方法 随机选取我院2023年2月至2024年2月收治的结节直径<20 mm的肺部疾病患者93例作为研究对象。所有患者均行DR诊断、DR双能减影(DES)诊断,以CT及CT引导下活检结果为金标准,对比2种诊断方式的检出率、良恶性结节的诊断效能以及图像质量。结果 DES诊断直径≤10 mm检出率、总检出率均高于DR诊断,差异有统计学意义(P<0.05)。DES鉴别诊断良恶性结节的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于DR诊断。DR诊断Kappa值=0.37,DES诊断Kappa值=0.86。DES诊断与肋骨重叠结节的图像质量高于DR诊断,差异有统计学意义(P<0.05);DES诊断与肋骨无重叠结节的图像质量与DR诊断相比,差异无统计学意义(P>0.05)。结论 与DR相比,DES诊断肺部小结节图像质量佳,不仅能提高诊断准确率,还有助于鉴别诊断良恶性结节。
Objective Explorethepreliminary judgment of small pulmonary nodulesby direct digital X-ray photography(digital radiography;DR)examination.Methods A total of 93 patients with pulmonary disease with nod-ule diameter<20 mm from February 2023 to February 2024 were selected.All patients were diagnosed with DR diag-nosis,DR double subtraction(dual energy subtraction;DES),and CT and CT guided biopsy results as the gold stan-dard to compare the detection rate of the two diagnostic methods,the diagnostic efficacy of benign and malignant nod-ules,and the image quality.Results The detection rate of DES diagnosis diameter 1Omm were higher than that of DR diagnosis,statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and neg-ative predictive value of DES differentiation diagnosis between benign and malignant nodules were all higher than DR diagnosis.DR diagnosis Kappa value=0.37 and DES diagnosis Kappa value=0.86.The image quality of DES diagno-sis and rib overlapping nodules was higher than DR diagnosis,statistically significant(P<0.05);DES diagnosis com-pared with DR diagnosis(P>0.05).Conclusion Compared with DR,DES can diagnose small pulmonary nodules,which can not only improve the diagnostic accuracy,but also contribute to the differential diagnosis of benign and ma-lignant nodules.
