HR-HPV感染宫颈病变患者阴道微生态及hs-CRP、TGF-β1的表达研究

Study of vaginal microbiota and expression of hypersensitive C-reactive protein and transforming growth factor-β1 in patients with high-risk human papilloma virus infected cervical lesions

摘要 目的分析高危型人乳头瘤病毒(HR-HPV)感染宫颈病变患者阴道微生态及超敏C反应蛋白(hs-CRP)、转化生长因子-β1(TGF-β1)的表达。方法选取2021年1月至2023年3月唐山市妇幼保健院收治的112例宫颈病变患者作为研究对象,根据病理诊断结果分为宫颈上皮内瘤变(CIN)组(n=60)和宫颈癌组(n=52),并选取同期收治的50例无人乳头瘤病毒(HPV)感染的良性宫颈病变患者作为对照组。比较三组阴道微生态、阴道炎检出情况、血清hs-CRP和TGF-β1表达水平,采用多因素Logistic回归分析HR-HPV感染宫颈病变的影响因素。结果宫颈癌组pH阳性率、过氧化氢(H_(2)O_(2))阳性率均高于CIN组、对照组,菌群密集度正常率、菌群多样性正常率、优势菌正常率均低于CIN组、对照组,CIN组pH阳性率、H_(2)O_(2)阳性率均高于对照组,菌群密集度正常率、菌群多样性正常率、优势菌正常率均低于对照组(P<0.05)。宫颈癌组细菌性阴道炎(BV)、外阴阴道假丝酵母菌病(VVC)检出率均高于CIN组、对照组,CIN组BV、VVC检出率均高于对照组(P<0.05)。宫颈癌组血清hs-CRP、TGF-β1水平均高于CIN组、对照组,CIN组血清hs-CRP、TGF-β1水平均高于对照组(P<0.05)。pH、优势菌异常、BV、hs-CRP、TGF-β1是HR-HPV感染宫颈病变的影响因素(P<0.05)。结论HR-HPV感染宫颈病变患者与阴道微生态失衡、BV发生及血清hs-CRP、TGF-β1表达显著相关。 Objective To analyze vaginal microbiota and the expression of hypersensitive C-reactive protein(hs-CRP)and transforming growth factor-β1(TGF-β1)in patients with high-risk human papilloma virus(HR-HPV)infected cervical lesions.Methods A total of 112 patients with cervical lesions who were treated in Tangshan Maternal and Child Health Hospital from January 2021 to March 2023 were collected as the study subjects.According to the pathological diagnosis,the patients were separated into a cervical intraepithelial neoplasia(CIN)group(n=60)and a cervical cancer group(n=52).A total of 50 patients with benign cervical lesions without human papilloma virus(HPV)infection admitted to hospital during the same period were selected as the control group.Vaginal microbiota,vaginitis detection condition,serum hs-CRP and TGF-β1 expression levels were compared among the three groups.Multivariate Logistic regression was applied to analyze the influencing factors of cervical lesions caused by HR-HPV infection.Results The pH positive rate and H_(2)O_(2) positive rate of the cervical cancer group were both higher than those of the CIN group and the control group,and the normal rate of bacterial population density,normal rate of microbial diversity,and normal rate of dominant bacteria were all lower than those of the CIN group and the control group,the pH positive rate and H_(2)O_(2) positive rate in CIN group were both higher than those in control group,and normal rate of bacterial population density,normal rate of microbial diversity,and normal rate of dominant bacteria were all lower than those in control group(P<0.05).The detection rates of bacteriad vaginosis(BV)and vulvovaginal candidiasis(VVC)were both higher in the cervical cancer group than in the CIN group and the control group,the detection rates of BV and VVC in the CIN group were both higher than those in the control group(P<0.05).The serum levels of hs-CRP and TGF-β1 in the cervical cancer group were both higher than those in the CIN group and the control group,and serum hs-CRP and TGF-β1 levels in the CIN group were both higher than in the control group(P<0.05).PH value,abnormal dominant bacteria,BV,hs-CRP,and TGF-β1 were the influencing factors of HR-HPV infection in cervical lesions(P<0.05).Conclusions Patients with HR-HPV infected cervical lesions are significantly correlated with vaginal microbiota imbalance,BV occurrence,serum hs-CRP and TGF-β1 expression.
