摘要
目的:在类似非典、新冠等新发传染性公共卫生事件下,医疗机构尤其是大型医院在短时间内会接收大量重症患者,本文旨在探索医疗机构短时间内快速实现对重症患者科学诊疗的有效模式和路径。方法:以新冠肺炎疫情为例,对四川大学华西医院在疫情应急和常态化背景下形成的新冠感染重症患者诊疗模式和实践经验进行总结和分析。结果:四川大学华西医院在疫情下形成了医务管理主导的新冠感染患者多学科诊疗精细化管理模式。该模式通过集中院内优质医疗资源和专家团队,制定了精细化多学科诊疗工作流程和诊疗规范,有效提高了新冠重症患者的诊疗救治能力、降低了重症患者的病死率。结论:精细化管理和多学科诊疗的有机结合对于实现重症患者的精准诊疗和全面管理提供了强有力的支持和保障,由此构建的多学科诊疗精细化管理模式对于国内其他医疗机构尤其是大型综合性医院在未来类似突发公共卫生事件中快速实现对重症患者科学诊疗具有重要参考意义。
Objective To explore the effective mode and path for medical institutions to quickly achieve scientific diagnosis and treatment of severe patients in public health emergency.Methods The case study method was used to the diagnosis and treatment mode and practical experience of severe patients with COVID-19 infection formed in the context of COVID-19 emergency and normalization in West China Hospital of Sichuan University were summarized and analyzed.Results Under the epidemic situation,a multi-disciplinary refined management model of diagnosis and treatment of COVID-19 infection patients dominated by medical management was formed in West China Hospital of Sichuan University,which has developed a refined multidisciplinary diagnosis,treatment workflow,diagnosis and treatment standards by concentrating hospital high-quality medical resources and expert teams,effectively improved the diagnosis and treatment ability of COVID-19 severe patients and reduced the mortality of severe patients.Conclusion The combination of refined management and multidisciplinary diagnosis and treatment provides strong support and guarantee for achieving the precise diagnosis and comprehensive management of severe patients.The multi-disciplinary refined management model constructed from which has important reference significance for other medical institutions in China,especially for large comprehensive hospitals,to quickly achieve scientific diagnosis and treatment of severe patients in similar public health emergencies in the future.
