摘要
目的分析感染性心内膜炎(IE)感染病原学特征及TGF-β转导途径在免疫调节中的作用。方法选取2022年2月-2023年1月于南京医科大学附属常州市第二人民医院心内科就诊的感染性心内膜炎患者35例为IE组,另选取健康体检者30名为对照组,分析IE组分离病原学特征,检测两组炎性因子指标、TGF-β转导相关蛋白表达及CD_(4)^(+)CD_(25)^(+)调节性T淋巴细胞水平。结果IE组35例患者共分离病原菌36株,其中革兰阴性菌11株(30.56%),革兰阳性菌24株(66.67%),真菌1株(2.78%);与对照组比较,IE组白细胞计数(WBC)、C-反应蛋白(CRP)、降钙素原(PCT)、白细胞介素(IL)-1β和IL-18表达水平升高(P<0.05),TGF-β1、Smad2、Smad3、p-Smad2和p-Smad3蛋白相对表达水平升高(P<0.05),CD_(4)^(+)CD_(25)^(+)水平降低(P<0.05)。结论感染性心内膜炎患者的TGF-β转导途径相关蛋白异常高表达,并与机体免疫调节有关,通过介导T淋巴细胞活化进而干预患者体内炎性因子水平。
OBJECTIVE To analyze the etiological characteristics of infectious endocarditis(IE)and observe the effect of TGF-βtransduction pathway on immunomodulatory regulation.METHODS A total of 35 patients with IE who were treated in cardiology department of Changzhou Second People′s Hospital Affiliated to Nanjing Medical University from Feb 2022 to Jan 2023 were assigned as the IE group,and 30 healthy peoples who received physical examination were chosen as the control group.The etiological characteristics of the IE group were analyzed.The levels of inflammatory factors,TGF-βtransduction-related proteins and CD_(4)^(+)+CD_(25)^(+)regulatory T lymphocytes were detected for the two groups.RESULTS Totally 36 strains of pathogens were isolated from the 35 IE patients,11(30.56%)of which were gram-negative bacteria,24(66.67%)were gram-positive bacteria,and 1(2.78%)was fungus.The expression levels of white blood cell(WBC),C-reactive protein(CRP),procalcitonin(PCT),interleukin(IL)-1βand IL-18 of the IE group were higher than those of the control group(P<0.05);the relative expression levels of TGF-β1,Smad2,Smad3,p-Smad2 and p-Smad3 proteins of the IE group were higher than those of the control group(P<0.05),while the CD_(4)^(+)+CD_(25)^(+)level of the IE group was lower than that of the control group(P<0.05).CONCLUSION The IE patients show the abnormal high expressions of TGF-βtransduction pathway-related proteins,which may be associated with the immunomodulatory regulation and intervene in the levels of inflammatory factors by mediating the activation of T lymphocytes.
