健康体检人群骨密度定量CT骨密度与HDL-C的相关性研究

Correlation between HDL-C and Quantitative CT Measurement of Bone Mineral Density in Healthy Physical Examination Populations

摘要 目的分析健康体检人群的高密度脂蛋白胆固醇(HDL-C)与骨密度(BMD)之间的关系,为骨质疏松防治提供指导。方法搜集河南省人民医院健康管理学科2018年9月至2023年2月17448例体检人群作为研究对象。所有研究对象均进行体格检查、血脂检测和定量CT(QCT)测量其腰椎BMD。采用Pearson和Spearman相关分析评估各指标与BMD的相关性,采用多元线性回归分析进一步分析HDL-C与BMD的关系。结果按BMD分组:骨质疏松组血清HDL-C平均值显著高于骨量减少组和骨量正常组(P<0.05)。回归分析调整其他变量后结果显示,HDL-C水平与腰椎BMD呈负相关(t=-4.364,P<0.05),且女性人群中的负相关更为显著(t=-6.858,P<0.05)。按体质量指数(BMI)分组:HDL-C水平与腰椎BMD在超重和肥胖人群中为负相关(r=-0.039,P<0.01;r=-0.126,P<0.01);而在BMI<24 kg/m^(2)组无相关,差异无统计学意义(P>0.05)。结论血清HDL-C水平的升高是骨质疏松发生的危险因素,在女性以及超重和肥胖人群更为突出,为骨质疏松的预防提供科学依据。 Objective To analyze the relationship between high-density lipoprotein cholesterol(HDL-C)and bone mineral density in healthy physical examination populations to provide guidance for the prevention and treatment of osteoporosis.Methods This study was a retrospective study that collected 17,448 physical examination people from September 2018 to February 2023 in the health management discipline of Henan Provincial People's Hospital.All participants underwent physical examination,lipid profile and quantitative CT(QCT)to measure their lumbar spine bone density.Pearson and Spearman correlation analysis was used to evaluate the correlation between each index and bone density,and multiple linear regression analysis was used to further analyze the relationship between HDL-C and bone density.Results According to bone mineral density,the mean value of serum HDL-C in the osteoporosis group was significantly higher than that in the osteopenia and normal bone mass group(P<0.05).After adjusting for other variables,the results of regression analysis showed that HDL-C levels were negatively correlated with lumbar spine bone mineral density(t=-4.364,P<0.05),and the negative correlation was more significant in the female population(t=-6.858,P<0.05).According to body mass index(BMI),HDL-C level was negatively correlated with lumbar spine bone mineral density in overweight and obese people(r=-0.039,P<0.01;r=-0.126,P<0.01).There was no correlation in the BMI<24 kg/m^(2)group,and the difference was not statistically significant(P>0.05).Conclusion Elevated serum HDL-C levels are risk factors for osteoporosis,and are more prominent in women and overweight and obese people,providing a scientific basis for the prevention of osteoporosis.
