无创通气联合牛肺表面活性剂治疗新生儿呼吸窘迫综合征的疗效及对患儿肺氧合功能、血气指标、肺循环功能的影响

摘要 目的 探究无创通气联合牛肺表面活性剂治疗新生儿呼吸窘迫综合征(NRDS)的疗效及对患儿肺氧合功能、血气指标、肺循环功能的影响。方法 选取80例NRDS患儿为研究对象,按治疗方法不同分成对照组(采用无创通气治疗, n=40)和研究组(采用无创通气联合牛肺表面活性剂治疗,n=40)。比较两组临床疗效、肺氧合功能[动脉血氧饱和度(SaO_(2))、氧合指数(PaO_(2)/FiO_(2))]、血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、肺循环功能[肺动脉压(PAP)、血管外肺水含量(EVLW)、肺血管阻力(PVR)]。结果 治疗后,研究组的总有效率95.00%高于对照组的80.00%(P<0.05)。治疗后,两组SaO_(2)、PaO_(2)/FiO_(2)均高于治疗前,且研究组SaO_(2)(95.37±3.52)%、PaO_(2)/FiO_(2)(295.66±30.35)mm Hg(1 mm Hg=0.133 kPa)均高于对照组的(91.62±3.43)%、(263.89±28.74)mm Hg(P<0.05)。治疗后,两组PaO_(2)高于治疗前、PaCO_(2)低于治疗前,且研究组PaO_(2)(81.71±6.37)mm Hg高于对照组的(73.24±6.33)mm Hg、PaCO_(2)(36.42±4.79)mm Hg低于对照组的(43.34±5.27)mm Hg(P<0.05)。治疗后,两组PAP、EVLW、PVR均低于治疗前,且研究组PAP(22.47±1.51)mm Hg、EVLW(5.56±1.62)ml/kg、PVR(22.47±5.67)kPa·s/L均低于对照组的(25.33±2.24)mm Hg、(8.41±2.55)ml/kg、(29.31±6.25)kPa·s/L(P<0.05)。结论 针对NRDS患儿,无创通气联合牛肺表面活性剂治疗具有显著疗效,能有效改善患儿的肺氧合功能、血气指标及肺循环功能。 Objective To explore the efficacy of noninvasive ventilation combined with bovine pulmonary surfactant on neonatal respiratory distress syndrome(NRDS)and its influence on pulmonary oxygenation,blood gas index and pulmonary circulatory function.Methods 80 children with NRDS were selected as research subjects and divided into a control group(noninvasive ventilation treatment,n=40)and a study group(noninvasive ventilation combined with bovine pulmonary surfactant treatment,n=40)according to different treatment methods.Comparison was made on clinical efficacy,pulmonary oxygenation function[arterial oxygen saturation(SaO_(2)),oxygenation index(PaO_(2)/FiO_(2))],blood gas index[arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))],pulmonary circulation function[pulmonary artery pressure(PAP),extravascular lung water(EVLW),pulmonary vascular resistance(PVR)between the two groups.Results After treatment,the total effective rate of 95.00%in the study group was higher than 80.00%in the control group(P<0.05).After treatment,SaO_(2),PaO_(2)/FiO_(2) in both groups were higher than those before treatment;the study group had SaO_(2) of(95.37±3.52)%and PaO_(2)/FiO_(2) of(295.66±30.35)mm Hg(1 mm Hg=0.133 kPa),which were higher than(91.62±3.43)%and(263.89±28.74)mm Hg in the control group(P<0.05).After treatment,PaO_(2) in both groups was higher than that before treatment,and PaCO_(2) was lower than that before treatment;PaO_(2) of(81.71±6.37)mm Hg in the study group was higher than(73.24±6.33)mm Hg in the control group,and PaCO_(2) of(36.42±4.79)mm Hg was lower than(43.34±5.27)mm Hg in the control group(P<0.05).After treatment,PAP,EVLW and PVR in both groups were lower than those before treatment;the study group had PAP of(22.47±1.51)mm Hg,EVLW of(5.56±1.62)ml/kg and PVR of(22.47±5.67)kPa·s/L,which were lower than(25.33±2.24)mm Hg,(8.41±2.55)ml/kg and(29.31±6.25)kPa·s/L in the control group(P<0.05).Conclusion For children with NRDS,non-invasive ventilation combined with bovine pulmonary surfactant has a significant effect,which can effectively improve the pulmonary oxygenation function,blood gas index and pulmonary circulation function of children.
