微格教学法在培训全科转岗医生门诊接诊能力中的应用

摘要 目的探讨微格教学法在全科转岗医生的门诊接诊能力培训中的效果。方法选取在西安市中心医院接受全科转岗培训的医师187名,应用微格教学法培训全科转岗医生的门诊接诊能力,分别于培训前后对病史采集、问诊技巧、体格检查、全科评估与处理、人文关怀和SOAP病历书写进行考核,评估教学效果。结果与培训前相比,培训后全科转岗医生病史采集、问诊技巧、体格检查、全科评估与处理、人文关怀和SOAP病历书写得分均显著提高(均P<0.05)。但按工作年限分组后,发现在SOAP病例书写方面,工作年限≥10年的全科转岗医生培训效果并不显著(P>0.05)。结论微格教学法可有效提高转岗医生的全科接诊能力、SOAP病历书写和全科思维能力,并可规范诊疗行为,提高自主学习动力。但工作年限≥10年者,在SOAP培训方面仍需探究更加有效的提升方法。 Objective To explore the effect of micro-teaching method in the training of outpatient reception ability of general practice transfer physicians.Methods A total of 187 physicians who received general practice transfer training in Xi'an Central Hospital were selected to be trained in the outpatient reception ability with micro-teaching method.Medical history collection,consultation skills,physical examination,general practice evaluation and processing,humanistic care and SOAP medical record writing were evaluated before and after the training,so as to evaluate the teaching effect.Results After training,compared with before training,their abilities in general medical history collection,inquiry skills,physical examination,general medical evaluation and processing,humanistic care and SOAP medical record writing were significantly improved(P<0.05).However,after grouping according to different working years,it was found that the training effect of working years≥10 years was not significant in SOAP medical record writing(P>0.05).Conclusions The application of micro-teaching method in the training of the outpatient reception ability of the job transfer training for general practitioners is significant,which can effectively improve the general practice reception ability,SOAP medical record writing and general thinking ability of the transferred doctors,and can standardize the diagnosis and treatment behavior and improve the self-learning motivation.But for the physicians with more years of service,more effective promotion methods are still needed to be explored in SOAP training.
