摘要
目的探讨超声检查在儿童胃肠道息肉诊断中的应用价值。方法选取超声检查诊断胃肠道息肉患儿681例,所有患儿均经消化道内镜、腹腔镜或开腹手术行息肉摘除术,将超声检查与手术、病理对照。结果超声检查诊断胃肠道息肉的总的敏感度、特异度、阳性预测值、阴性预测值、Youden指数及准确度均在97.00%以上,Kappa值为0.975,P<0.001,超声检查诊断与术后病理对照具有高度一致性。息肉病理类型最多见的是幼年性息肉647例(95.00%)、其次Peutz-Jeghers综合征肠息肉25例(3.67%)、幼年性息肉病4例(0.59%),三种病理类型的胃肠道息肉均具有典型的临床特征和声像图表现。结论不同病理类型的儿童胃肠道息肉声像图各具特点,超声检查诊断与病理具有高度一致性,可为临床诊断和治疗提供可靠的影像学依据。
Objective To investigate the value of ultrasonography in the diagnosis of gastrointestinal polyps in children.Methods A total of 681 children with gastrointestinal polyps were diagnosed by ultrasound.All the children underwent endoscopic,laparoscopic or open surgery to remove polyps.The findings of ultrasound were compared with surgery and pathology.Results The overall sensitivity,specificity,PPV,NPV,Youden index and accuracy of ultrasonic diagnosis of gastrointestinal polyps were above 97.00%,Kappa value was 0.975,P<0.001,and ultrasound diagnosis was highly consistent with postoperative pathological results.The most common pathological types of polyps were juvenile polyposis in 647 cases(95.00%),followed by intestinal polyps with Peutz-Jeghers syndrome in 25 cases(3.67%)and juvenile polyposis syndrome in 4 cases(0.59%).The three types showed typical ultrasonographic manifestations.Conclusions The ultrasonography of gastrointestinal polyps in children with different pathological types has its own characteristics,and the ultrasonography diagnosis and pathological results are highly consistent,which can provide reliable imaging basis for clinical diagnosis and treatment.
