膝骨性关节炎患者应用本体感觉训练联合等速运动治疗对其步行能力的影响观察

Observation on the Effect of Proprioceptive Training Combined with Isokinetic Exercise on Walking Ability in Patients with Knee Osteoarthritis

摘要 目的 探究膝关节骨性关节炎(KOA)患者应用本体感觉训练联合等速运动治疗对其步行能力的影响。方法 选取2021年6月至2022年9月河南省直第三人民医院医学运动科治疗的90例KOA患者为研究对象,随机数表法分为三组,各30例,A组采取物理因子治疗与常规运动治疗,B组采取物理因子治疗与本体感觉训练治疗,C组在B组基础上加等速肌力训练治疗,于治疗前,治疗4周后,使用等速测试仪测量三组治疗前后的峰值力矩(PT)、主动角度重现测试值(AAR)及被动运动阈值测量值(TDPM);使用平衡训练仪检测三组治疗前后评估完成耗时(TTE)和平均轨迹误差(ATE),记录起立—行走计时测试(TUGT),观察患者步行能力,并用视觉模拟评分法(VAS)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)量表分别评价治疗前后膝关节疼痛与生活质量。结果 治疗前,三组患者PT、AAR、TDPM、TTE、ATE、VAS、WOMAC、TUGT水平差异无统计学意义(P> 0.05);三组治疗后PT高于治疗前,AAR、TDPM、TTE、ATE、VAS、WOMAC、TUGT水平均明显低于治疗前,差异均有统计学意义(P <0.05);治疗后组间比较,B、C两组PT高于A组,且C组高于B组,差异均有统计学意义(P <0.05);B、C两组AAR、TDPM、TTE、ATE、VAS、WOMAC、TUGT水平均低于A组,且C组低于B组,差异均有统计学意义(P <0.05)。结论 应用本体感觉训练联合等速运动治疗KOA患者效果更好,对患者关节功能、步行能力改善明显,可缓解患者膝盖疼痛及提高生活质量。 Objective To explore the effect of proprioceptive training combined with isokinetic exercise on the walking ability of patients with knee osteoarthritis(KOA).Methods Ninety KOA patients treated at the Department of Medical Sports,The Third People’s Hospital of Henan Province from June 2021 to September 2022 were selected as study subjects.They were randomly divided into three groups of 30 patients each using the random number table method.Group A received physical factor therapy and conventional exercise therapy,Group B received physical factor therapy and proprioceptive training,and Group C received proprioceptive training combined with isokinetic muscle strength training based on Group B.Before and after 4 weeks of treatment,the peak torque(PT),active angle reproduction(AAR),and threshold to detect passive motion(TDPM)were measured using an isokinetic tester.The time to complete the evaluation(TTE)and average trajectory error(ATE)were measured using a balance trainer.The timed up and go test(TUGT)was recorded to observe the patients’walking ability.The visual analog scale(VAS)and Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)were used to evaluate knee pain and quality of life before and after treatment.Results Before treatment,there were no statistically significant differences in PT,AAR,TDPM,TTE,ATE,VAS,WOMAC,and TUGT levels among the three groups(P>0.05).After treatment,PT levels in all three groups were higher than before treatment,while AAR,TDPM,TTE,ATE,VAS,WOMAC,and TUGT levels were significantly lower than before treatment,with statistically significant differences(P<0.05).After treatment,PT levels in Groups B and C were higher than in Group A,and PT levels in Group C were higher than in Group B,with statistically significant differences(P<0.05).AAR,TDPM,TTE,ATE,VAS,WOMAC,and TUGT levels in Groups B and C were lower than in Group A,and levels in Group C were lower than in Group B,with statistically significant differences(P<0.05).Conclusion The application of proprioceptive training combined with isokinetic exercise in KOA patients has a better effect,significantly improves joint function and walking ability,relieves knee pain,and improves quality of life.
