摘要
目的分析研究血清微小RNA-23a(miR-23a)、白细胞介素-6(IL-6)与老年慢性心力衰竭(CHF)患者认知功能障碍的关系。方法选取2023年5月至2023年12月在渭南市中心医院接受治疗的老年CHF合并认知功能障碍的患者为研究对象,将其命名为病例组(n=102),另选同期治疗的无认知功能障碍老年CHF患者为对照组(n=80)。比较两组血清miR-23a、IL-6水平;对影响老年CHF患者认知功能障碍的因素进行多因素Logistic回归分析;绘制ROC曲线评估miR-23a、IL-6对老年CHF患者发生认知功能障碍的预测价值。结果病例组的血清miR-23a水平低于对照组,IL-6水平高于对照组(t=6.612、5.469,均P<0.05);多因素Logistic回归分析显示,年龄为75~85岁(OR=2.040)、文化程度为初中及以下(OR=1.881)、血清miR-23a低表达(OR=2.179)及IL-6水平升高(OR=2.102)均是老年CHF患者发生认知功能障碍的独立危险因素(P<0.05);ROC曲线分析显示,血清miR-23a、IL-6水平及联合检测的曲线下面积(AUC)分别为0.762、0.723、0.815,联合检测优于单一检测(P<0.05)。结论血清miR-23a、IL-6水平与老年CHF患者发生认知功能障碍相关。
Objective To analyze the relationship between serum microRNA-23a(miR-23a),in-terleukin-6(IL-6)and cognitive dysfunction in elderly patients with chronic heart failure(CHF).Methods Elderly patients with CHF combined with cognitive dysfunction who received treatment at Weinan Central Hos-pital from May 2023 to December 2023 were selected as the study subjects and referred to as the case group(n=102),and elderly CHF patients without cognitive impairment treated during the same period were selected as the control group(n=80).The serum levels of miR-23a and IL-6 were compared between the two groups.The influencing factors of cognitive dysfunction in elderly patients with CHF were analyzed by multivariate lo-gistic regression analysis,and ROC curve was drawn to analyze the predictive value of serum miR-23a and IL-6 levels on cognitive dysfunction in elderly CHF patients.Results The serum miR-23a level in the case group was lower than that in the control group,and the serum IL-6 level was higher than that in the control group(t=6.612,5.469,P<0.05).Multivariate logistic regression analysis showed that age 75-85 years old(OR=2.040),education level of junior high school OR below(OR=1.881),low expression of serum miR-23a(OR=2.179),and elevated IL-6 level(OR=2.102)were independent risk factors for cognitive dysfunction in elderly CHF pa-tients(P<0.05).ROC curve analysis showed that the levels of serum miR-23a and IL-6 and the AUC of com-bined detection were 0.762,0.723 and 0.815,respectively.Combined detection was superior to single detec-tion(P<0.05).Conclusion Serum miR-23a and IL-6 levels are associated with cognitive dysfunction in el-derly patients with CHF.
