布地奈德联合乙酰半胱氨酸雾化吸入治疗慢性阻塞性肺疾病急性加重期患者的效果

摘要 目的:探究布地奈德联合乙酰半胱氨酸雾化吸入治疗慢性阻塞性肺疾病急性加重期(AECOPD)患者的效果。方法:前瞻性选取2017年1月—2022年12月中国人民解放军联勤保障部队第九〇〇医院仓山院区收治的80例AECOPD患者。随机将其分为观察组和对照组,各40例。对照组实施乙酰半胱氨酸雾化吸入,观察组在对照组基础上给予布地奈德雾化吸入。比较两组临床疗效,治疗前后肺功能、血气指标、炎症因子及不良反应。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组最大呼气流量(PEF)、第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC%均升高,观察组PEF、FEV_(1)、FVC、FEV_(1)/FVC%均高于对照组,差异有统计学意义(P<0.05)。治疗后,两组动脉血二氧化碳分压(PaCO_(2))降低,血氧饱和度(SaO_(2))、氧分压(PaO_(2))均升高,观察组PaCO_(2)低于对照组,SaO_(2)、PaO_(2)均高于对照组,差异有统计学意义(P<0.05)。治疗后,两组C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-1β(IL-1β)水平均降低,观察组CRP、PCT、IL-1β水平均显著低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:布地奈德联合乙酰半胱氨酸雾化吸入治疗AECOPD患者临床效果显著,可提高患者肺功能水平,改善血气,减轻炎症反应,且具有一定安全性。 Objective:To investigate the effect of Budesonide combined with Acetylcysteine atomized inhalation in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Method:A total of 80 patients with AECOPD admitted to Cangshan Branch,900 Hospital of Joint Logistics Support Force from January 2017 to December 2022 were prospectively selected.They were randomly divided into observation group and control group,40 cases in each group.The control group was given atomized inhalation of Acetylcysteine,and the observation group was given atomized inhalation of Budesonide on the basis of the control group.The clinical efficacy,lung function,blood gas index,inflammatory factors before and after treatment and adverse reactions were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the maximum expiratory flow rate(PEF),forced expiratory volume in the first second(FEV_(1)),forced vital capacity(FVC)and FEV_(1)/FVC%in both groups were increased,and the PEF,FEV_(1),FVC and FEV_(1)/FVC%in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,the arterial partial pressure of carbon dioxide(PaCO_(2))was decreased,and the oxygen saturation(SaO_(2))and oxygen partial pressure(PaO_(2))were both increased in both groups,PaCO_(2) in the observation group was lower than that in the control group,and SaO_(2) and PaO_(2) were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of C-reactive protein(CRP),procalcitonin(PCT)and interleukin-1β(IL-1β)were decreased in both groups,and the levels of CRP,PCT and IL-1βin observation group were significantly lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Budesonide combined with Acetylcysteine atomized inhalation in the treatment of AECOPD patients has significant clinical effect,which can improve the lung function level of patients,improve blood gas,reduce inflammation,and has a certain safety.
