摘要
目的 探讨个案管理模式下代谢综合征患者行减重代谢手术前后的人体成分变化。方法 选取宁夏医科大学总医院胃肠外科2021年11月至2022年5月住院行减重代谢手术的患者,采用科室自行制定的个案管理模式对患者进行术后照护与随访,并对其手术前后的人体成分进行检测,观察患者不同时期部分人体参数的变化情况。结果 66例减重代谢手术患者经2个月术后体系化的个案管理后,将手术前与手术后的人体成分相比,患者的骨骼肌肉量和肌肉含量降低,部分营养物质在合理范围内有所减少,颈围、胸围、腹围、臀围变小,躯干、双下肢浮肿指数均降低(P均<0.05),患者左、右上肢的浮肿参数变化不明显(P>0.05)。结论 将个案管理模式应用于减重代谢术后患者的照护,有助于促进肥胖患者身体围度减小、浮肿指数降低。
Objective To explore the changes of body composition of patients with metabolic syndrome before and after metabolic and bariatric surgery under the case management mode.Methods The patients who were hospitalized for metabolic and bariatric surgery from November 2021 to May 2022 in the department of gastrointestinal surgery in General Hospital of Ningxia Medical University were enrolled.After surgery,a case management model independently developed by the department was adopted to provide postoperative care and follow-up for patients,and the body composition of patients before and after surgery were tested to observe the changes in some body parameters of patients at different stages.Results Two months after the systematic case management of 66 patients with metabolic and bariatric surgery,compared with the preoperative and postoperative body composition,the skeletal muscles and muscle content of the patients were significantly reduced,and some nutrients were reduced within a reasonable range.The neck circumference,chest circumference,abdominal circumference,and hip circumference were significantly reduced,and the swelling index of the trunk and lower limbs was reduced(P all<0.05).However,there was no significant change in the swelling parameters of the left and right upper limbs of the patients(P>0.05).Conclusion Applying the case management model to the care of patients after weight-loss metabolism surgery can help promote a reduction in body circumference and swelling index in obese patients,so it can be used as an auxiliary care for patients after weight-loss metabolism surgery.
