摘要
目的 探讨中医雷火灸治疗中期股骨头缺血性坏死患者的临床效果及对髋关节功能的影响。方法 回顾性分析南昌市洪都中医院2020年5月—2022年5月收治的中期股骨头缺血性坏死患者70例,按照治疗方案分组,对照组34例给予常规西药治疗,研究组36例给予常规西药结合中医雷火灸治疗。治疗4周后,评估两组临床疗效、中医证候积分、血清指标,并以Harris髋关节评分量表为依据评估髋关节功能情况。结果 治疗后,研究组关节沉重、髋部疼痛、舌质暗、脉弦涩积分均低于对照组(P<0.05);肿瘤坏死因子-α(TNF-α)水平低于对照组,血管内皮生长因子(VEGF)水平、Harris髋关节评分均高于对照组(P<0.05)。研究组治疗总有效率为97.22%(35/36),高于对照组的82.35%(28/34)(P<0.05)。结论 为中期股骨头缺血性坏死患者提供中医雷火灸治疗,可降低其中医证候积分,并改善血清指标,对髋关节功能的改善有重要意义,临床效果确切。
Objective To investigate the clinical effect of thunder-fire moxibustion in treating metaphase avascular necrosis of femoral head and its influence on hip joint function.Methods Retrospective analysis was performed on 70 patients with metaphase avascular necrosis of femoral head admitted to Nanchang Hongdu Hospital of Traditional Chinese Medicine from May 2020 to May 2022,and they were divided into groups according to the treatment plan.A total of 34 patients treated with conventional Western medicine were included in the control group,and 36 patients treated with conventional Western medicine and thunder-fire moxibustion were in-cluded in the study group.After 4 weeks of treatment,the clinical efficacy of the two groups was evaluated,and the TCM symptom scores and serum indexes were compared.At the same time,the hip function of the two groups was evaluated based on the Harris hip Score scale.Results After treatment,the scores of joint heaviness,hip pain,dark tongue and astringent pulse in the study group were lower than those in the control group(P<0.05).The level of tumor necrosis fact-α(TNF-α)was lower than that of control group,and the level of vascular endothelial growth factor(VEGF)and Harris hip score were higher than those of control group(P<0.05).The total effective rate of the study group was 97.22%(35/36),which was higher than that of the control group(82.35%,28/34)(P<0.05).Con-clusion Thunder-fire moxibustion in the treatment of metaphase avascular necrosis of femoral head can reduce TCM syndrome score,and improve the serum index,which is of great significance for the improvement of hip joint function.
