摘要
目的探讨同型半胱氨酸(Hcy)、抗凝血酶Ⅲ(ATⅢ)、抗心磷脂抗体(ACA)与下肢深静脉血栓形成后综合征(PTS)患者预后的相关性及其预测价值。方法收集2020年1月至2023年1月于山西白求恩医院(山西医学科学院同济山西医院)山西医科大学第三医院接受治疗的98例下肢深静脉PTS患者的临床资料,根据患者预后情况将其分为预后良好组(n=70)和预后不良组(n=28),收集所有患者的临床资料,观察Hcy、ATⅢ、ACA与下肢深静脉PTS患者预后之间的关系,分析各指标对下肢深静脉PTS患者预后的预测价值。结果预后良好组患者Hcy、ACA水平均低于预后不良组患者,ATⅢ水平高于预后不良组患者,差异均有统计学意义(P﹤0.05)。Hcy、ACA指标均与下肢深静脉PTS患者预后呈正相关(r=0.513、0.493,P﹤0.05),ATⅢ与下肢深静脉PTS患者预后呈负相关(r=-0.504,P﹤0.05)。受试者工作特征(ROC)曲线显示,Hcy、ATⅢ、ACA对预测下肢深静脉PTS患者预后均具有较高的诊断价值,曲线下面积(AUC)分别为0.833、0.805、0.864,特异度分别为0.686、0.750、0.800,灵敏度分别为0.929、0.729、0.857。结论Hcy、ATⅢ及ACA均与下肢深静脉PTS患者预后显著相关,具有较高临床预测价值,能够有效筛查高危群体,为积极采取干预措施提供指导。
Objective To investigate the correlation and predictive value of homocysteine(Hcy),antithrombinⅢ(ATⅢ)and anticardiolipin antibody(ACA)of prognosis in patients with deep vein post-thrombotic syndrome(PTS)of lower extremity.Method Clinical data of 98 patients with deep vein PTS of lower extremity who were treated in Shanxi Bethune Hospital(Tongji Shanxi Hospital,Shanxi Academy of Medical Sciences),the Third Hospital of Shanxi Medical University from January 2020 to January 2023 were collected.According to the prognosis of the patients,they were divided into good prognosis group(n=70)and poor prognosis group(n=28).Clinical data of all patients were collected to observe the relationship between Hcy,AT Ⅲ,ACA and prognosis of deep vein PTS of lower extremity patients,the predictive value of each index for prognosis of deep vein PTS of lower extremity patients were analyzed.Result Hcy and ACA levels in the good prognosis group were lower than those in the poor prognosis group,AT Ⅲ level was higher than that in the poor prognosis group,with statistical significance(P<0.05).Both Hcy and ACA indexes were positively correlated with the prognosis of deep vein PTS of lower extremity patients(r=0.513,0.493,P<0.05),while AT Ⅲ was negatively correlated with the prognosis of deep vein PTS of lower extremity patients(r=-0.504,P<0.05).Receiver operating characteristics(ROC)curve showed that Hcy,AT Ⅲ and ACA had high diagnostic value in predicting the prognosis of deep vein PTS of lower extremity patients,the area under the curve(AUC)was 0.833,0.805 and 0.864,the specificity was 0.686,0.750 and 0.800,the sensitivities was 0.929,0.729 and 0.857,respectively.Conclusion Hcy,ATⅢand ACA are significantly correlated with the prognosis of deep vein PTS of lower extremity patients,and have high clinical predictive value,which can effectively screen high-risk groups and provide guidance for active intervention measures.
