摘要
糖尿病微血管病变是一类常见的糖尿病慢性并发症。有研究表明,血糖变异性(Glycemic variability,GV)可能是影响糖尿病微血管并发症的独立危险因素。本文结合近年来国内外研究文献,从血糖变异性影响糖尿病微血管并发症的机制以及改善血糖变异性的措施2大方面进行综述。文献复习结果表明,高水平的血糖变异性通过激活氧化应激通路、加重炎症及促进细胞凋亡等方面综合导致糖尿病微血管并发症的发生发展。阐明该作用机制对防治糖尿病微血管病变有重要意义,为改善血糖控制提供理论依据。
Diabetic microangiopathy is a common class of chronic complications of diabetes.Some studies have shown that blood glucose variability may be a risk factor that independently affects the diabetes complications.This paper reviews the mechanisms by which glycemic variability affects diabetic microvascular complications and the measures to improve glycemic variability in light of the domestic and international research literature in recent years.The results of the literature review indicate that high-level blood glucose variability leads to the development of diabetic microvascular complications by activating the oxidative stress pathway,aggravating inflammation and promoting apoptosis.Therefore,elucidating this mechanism of action is of great significance in the prevention and treatment of diabetic microangiopathy and provides a theoretical basis for improving glycemic control.
