红细胞压积、血红蛋白与老年心力衰竭患者心脏结构的关系

摘要 目的分析红细胞压积、血红蛋白与老年心力衰竭患者心脏结构的关系。方法选择2020年2月至2023年3月于泰州市第四人民医院收治的97例老年心力衰竭患者作为研究对象,入院前测定红细胞压积与血红蛋白,根据患者是否发生心脏结构改变分为发生组(n=45)与非发生组(n=52)。比较两组患者的红细胞压积、血红蛋白及临床资料,多因素Logistic回归分析老年心力衰竭患者发生心脏结构改变的影响因素。结果发生组红细胞压积、血红蛋白均低于未发生组(P<0.05)。发生组N末端脑钠肽前体(NT-proBNP)水平高于未发生组(P<0.05),两组年龄、性别、吸烟史、饮酒史、家族遗传病、糖尿病、收缩压(SBP)、舒张压(DBP)、心功能分级、体质指数(BMI)、血清白蛋白(ALB)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)、总胆固醇(TC)、合并基础疾病[高血压、冠状动脉粥样硬化性心脏病(冠心病)、心房颤动、慢性阻塞性肺疾病]、高密度脂蛋白胆固醇(HDL-C)比较,差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,NT-proBNP、红细胞压积、血红蛋白为老年心力衰竭患者发生心脏结构改变的影响因素(P<0.05)。结论红细胞压积、血红蛋白与老年心力衰竭患者心脏功能改变密切相关。 Objective To analyze the relationship among hematocrit(HCT),hemoglobin(Hb)and heart structure in elderly patients with heart failure(HF).Methods The elderly patients with HF(n=97)were chosen from the Fourth People’s Hospital of Taizhou City from Feb.2020 to Mar.2023.The levels of HCT and Hb were detected in all patients before hospitalization,and then they were divided,according to changes of heart structure,into change group(n=45)and non-change group(n=52).The levels of HCT and Hb and clinical materials were compared between 2 groups.The influence factors of heart structure changes were analyzed by using multi-factor Logistic regression analysis.Results The levels of HCT and Hb were lower in change group than those in non-change group(P<0.05).The levels of N-terminal pro-brain natriuretic peptide(NT-proBNP)was higher in change group than that in non-change group(P<0.05).The differences in age,gender,smoking history,drinking history,family family genetic disease,diabetes,systolic blood pressure(SBP),diastolic blood pressure(DBP),heart function classification,body mass index(BMI),serum albumin(ALB),low-density lipoprotein-cholesterol(LDL-C),triglyceride(TG),total cholesterol(TC),complicated basic disease[hypertension,coronary heart disease(CHD),atrial fibrillation(AF)],chronic obstructive pulmonary disease(COPD)and high-density lipoprotein-cholesterol(HDL-C)had no statistical significance between 2 groups(P>0.05).The results of multi-factor Logistic regression analysis showed that NT-proBNP,HCT and Hb were influence factors of heart structure changes in elderly HF patients(P<0.05).Conclusion HCT and Hb are closely correlated to heart structure changes in elderly HF patients.
