膝关节前内侧骨关节炎患者应用不同类型股骨髁假体单髁置换的疗效

Comparison of therapeutic effects of using different types of femoral condylar prostheses for unicompartmental knee arthroplasty in patients with anterior medial knee osteoarthritis

摘要 目的分析膝关节前内侧骨关节炎(OA)患者应用不同类型股骨髁假体单髁置换(UKA)的疗效。方法前瞻性纳入2019年1月至2022年12月收治的拟行UKA治疗的膝关节前内侧OA患者180例为研究对象,按随机号码表法分为观察组和对照组,每组90例。2组患者均行UKA治疗,对照组使用骨水泥型单髁假体,观察组使用全生物型单髁假体。记录并比较2组患者手术时间、术中出血量、引流量、术后首次下地时间,分别于术前、术后3个月,使用牛津膝关节功能评分(OKS)、数字模拟疼痛评分(VAS)评价患者膝关节功能与疼痛情况,同时测定患者的膝关节活动度,记录术后不良事件发生情况。结果观察组患者手术时间短于对照组(P<0.05);2组术中出血量、伤口引流量及术后首次下地时间比较,差异无统计学意义(P>0.05)。术后3个月,2组患者OKS评分及膝关节活动度较术前升高,VAS评分较术前降低(P<0.05),但组间比较差异均无统计学意义(P>0.05);观察组不良事件发生率低于对照组(P<0.05),2组生理性透亮线发生率比较,差异无统计学意义(P>0.05)。结论膝关节前内侧OA患者行UKA治疗中,无论使用骨水泥型单髁假体或全生物型单髁假体,在改善患者膝功能、术后疼痛方面短期效果相同,但与骨水泥型单髁假体相比,全生物型单髁假体手术时间短,术后不良事件少。 Objective To analyze the therapeutic effect of different types of femoral condyle prosthesis for unicompartmental knee arthroplasty(UKA)in patients with anterior medial knee osteoarthritis(OA).Methods This was a prospective study involving 180 patients with anterior medial knee OA admitted to our Hospital from January 2019 to December 2022 and subjected to an elective UKA.They were randomly divided into the observation group and control group,with 90 patients in each group using a random number table.Patients in the control group received bone cement type single condylar prosthesis for UKA and those in the observation group received all biological types of single condylar prosthesis.Operation time,intraoperative blood loss,drainage volume,and first time of postoperative off-bed activities were recorded and compared between the two groups.The Oxford Knee Score(OKS)and the Visual Analogue Scale(VAS)were used to evaluate knee function and pain before surgery and 3 months after surgery.Range of motion of the knee and postoperative adverse events were recorded.Results The operation time of the observation group was significantly shorter than that of the control group(P<0.05).There were no significant differences in intraoperative bleeding volume,wound drainage volume,and first time of postoperative off-bed activities between the two groups(P>0.05).Three months after surgery,the OKS score and range of motion of the knee significantly improved after surgery compared to those before surgery,while the VAS score significantly decreased(P<0.05).However,there were no significant differences in the OKS score,range of motion of the knee and VAS score postoperatively between groups(P>0.05).The incidence of adverse events in the observation group was significantly lower than that of the control group(P<0.05),and there was no significant difference in the incidence of physiological transparent lines between the two groups(P>0.05).Conclusion In patients with anterior medial OA of the knee joint undergoing UKA,the use of either bone cement type or all biological types of single condyle implant provides the same short-term effect on improving knee function and postoperative pain.However,compared with bone cement type single condyle implants,all biological types of single condyle implants have shorter operation time and fewer postoperative adverse events.
