摘要
目的分析研究系统性红斑狼疮患者应用免疫学检验联合检测诊断价值。方法随机选取南阳市第二人民医院在2022年3月至2023年12月进行诊治的80例系统性红斑狼疮患者作为研究对象。所有患者均进行抗核抗体(ANA)、抗双链DNA抗体(抗ds-DNA)和抗可提取核抗原抗体(抗ENA)单一检验以及三种联合检验,以临床综合评估结果为金标准,观察患者单一检验及联合检验诊断结果及诊断效能。结果临床综合评估结果中阳性患者70例,阴性患者10例。单一ANA检测中真阳性患者51例,真阴性患者1例;单一抗ds-DNA检测中真阳性患者47例,真阴性患者2例;单一抗ENA检测中真阳性患者51例,真阴性患者3例;三种联合检测中真阳性患者66例,真阴性患者8例。ANA、抗ds-DNA、抗ENA联合检验准确度、敏感度、特异度、阳性预测值、阴性预测值均高于单一ANA、抗ds-DNA、抗ENA检验,差异有统计学意义(P<0.05)。结论免疫学检验在系统性红斑狼疮诊断中具有重要的临床应用价值,联合检测免疫学指标可以显著提高疾病诊断准确性和可靠性。
Objective To analyze the diagnostic value of combined immunological testing in patients with systemic lupus erythematosus.Methods A total of 80 Systemic lupus erythematosus patients treated at Nanyang Second People's Hospital from March 2022 to December 2023 were randomly selected as study subjects.All patients underwent individual tests for antinuclear antibodies(ANA),anti-double-stranded DNA antibodies(anti-dsDNA),and anti-extractable nuclear antigen antibodies(anti-ENA),as well as combined testing of these markers.Clinical comprehensive assessment results were used as the gold standard to observe the diagnostic results and efficiency of individual and combined tests.Results Among the clinical comprehensive assessment results,70 patients were positive and 10 were negative.In individual ANA testing,there were 51 true-positive and 1 true-negative patients;in individual anti-dsDNA testing,there were 47 true-positive and 2 true-negative patients;and in individual anti-ENA testing,there were 51 true-positive and 3 true-negative patients.In combined testing,there were 66 true-positive and 8 true-negative patients.The accuracy,sensitivity,specificity,positive predictive value,and negative predictive value of the combined ANA,anti-dsDNA,and anti-ENA tests were higher than those of the individual tests,with statistical significance(P<0.05).Conclusion Immunological testing has important clinical application value in the diagnosis of Systemic lupus erythematosus.Combined testing of immunological indicators can significantly improve the accuracy and reliability of disease diagnosis.
