以需求为导向的分阶段健康教育在老年下肢深静脉血栓形成患者中的应用效果

摘要 目的:观察以需求为导向的分阶段健康教育在老年下肢深静脉血栓形成(DVT)患者中的应用效果。方法:选取2020年1月至2021年10月该院收治的80例老年下肢DVT患者进行前瞻性研究,采用信封法将其分为对照组和观察组各40例。对照组给予常规护理,观察组在对照组基础上给予以需求为导向的分阶段健康教育,比较两组护理前后负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、疼痛程度[视觉模拟评分法(VAS)]、自护能力[自我护理能力测定表(ESCA)]和健康行为[健康促进生活方式量表-Ⅱ(HPLP-Ⅱ)]评分。结果:护理后,两组SAS、SDS评分均低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05);护理1周后,两组VAS评分均低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05);护理后,两组ESCA和HPLP-Ⅱ评分均高于护理前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:在常规护理基础上给予以需求为导向的分阶段健康教育可提高老年下肢DVT患者自护能力和健康行为评分,降低负性情绪和疼痛程度评分,效果优于单纯常规护理。 Objective:To observe application effects of demand-oriented staged health education in elderly patients with lower extremity deep venous thrombosis(DVT).Methods:A prospective study was conducted on 80 elderly patients with lower extremity DVT admitted to the hospital from January 2020 to October 2021.They were divided into control group and observation group by using the envelope method,40 cases in each group.The control group was given routine nursing,while the observation group was given the demand-oriented staged health education on the basis of that of the control group.The scores of negative emotions[self-rating anxiety scale(SAS),self-rating depression scale(SDS)],the pain degree[visual analogue scale(VAS)],the self-care ability[exercise of self-care agency scale(ESCA)],and the health behavior[health promoting lifestyle profile-Ⅱ(HPLP-Ⅱ)]were compared between the two groups before and after the nursing.Results:After the nursing,the scores of SAS and SDS in the two groups were lower than those before the nursing,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 1 week of the nursing,the VAS scores of the two groups were lower than those before the nursing,that in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).Further,after the nursing,the ESCA and HPLP-Ⅱ scores of the two groups were higher than those before the nursing,those of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusions:On the basis of the routine nursing,the demandoriented staged health education can improve the self-care ability and the health behavior scores of the elderly patients with lower extremity DVT,and reduce the scores of negative emotion and pain degree.Moreover,it is superior to simple routine nursing.
