摘要
目的 本研究通过分析江苏农村地区远程心电网络平台的建设现状,探讨农村“心电一张网”建设的可行性。方法 选取江苏省睢宁县进行农村“心电一张网”平台建设,对入选村卫生室配置心电图机并对村医进行系统培训。比较培训早期及后期心电图数量及质量,并分析“心电一张网”覆盖地区ST段抬高型心肌梗死(STEMI)、心房颤动等疾病的筛查情况。结果 本研究“心电一张网”覆盖135所村卫生室。(1)通过系统性村医心电培训后,有效心电图率上升(90.8%vs 25.6%,P<0.000 1),重复、信息缺失、错误及干扰过大心电图比例均明显下降(0.6%vs 6.3%,2.0%vs 64.4%,0.2%vs 7.4%,6.4%vs 11.7%,P均<0.000 1)。(2)“心电一张网”筛查23 203例人群,其中STEMI 37例,心房颤动/心房扑动815例(3.51%)。结论 农村“心电一张网”建设可行性、操作性强,通过筛查早期诊断STEMI及心房颤动等疾病,对农村地区心血管疾病管理具有重要价值。
Objective To explore the feasibility of constructing a rural"ECG Network"by analyzing the current status of the remote ECG network platform in rural areas of Jiangsu.Methods Suining County in Jiangsu Province was selected for the construction of the rural"ECG Network"platform.ECG machines were provided to selected vil-lage clinics,and village doctors received systematic training.The quantity and quality of ECGs were compared be-tween the early and later stages of training.The screening of ST-segment elevation myocardial infarction(STEMI)and atrial fibrillation in the areas covered by the"ECG Network"was also analyzed.Results The study's"ECG Network"covered 135 village clinics.After systematic ECG training for village doctors,the rate of effective ECGs significantly increased(90.8%vs 25.6%,P<0.0001).The proportions of repeated,incomplete,erroneous,and highly interfered ECGs significantly decreased(0.6%vs 6.3%,2.0%vs 64.4%,0.2%vs 7.4%,6.4%vs 11.7%,all P<0.0001).@The"ECG Network"screened 23203 individuals,identifying 37 cases of STEMI and 815 cases(3.51%)of atrial fibrillation/flutter.Conclusion The construction of a rural"ECG Network"is highly feasible and operable.It holds significant value for the early diagnosis and management of cardiovascular diseases like STEMI and atrial fibrillation in rural areas.
