摘要
目的探讨窝沟封闭剂联合氟化物涂膜在预防儿童第一恒磨牙龋齿中的应用效果。方法随机选取2021年8月—2022年7月泰安市口腔医院口腔体检的120例儿童为研究对象,根据不同治疗方法分为两组,各60例。对照组仅采用窝沟封闭剂对咬合面较深的窝沟进行处理,观察组则在对照组的基础上增用氟化物涂膜进行预防性治疗。比较两组治疗后的6个月、12个月与18个月的封闭剂保存状况、患龋率龋均值以及不良事件的发生情况。结果封闭后18个月,观察组患龋率为1.61%(1/62),低于对照组的14.89%(7/47),差异有统计学意义(χ^(2)=5.118,P<0.05)。封闭后12个月和18个月,观察组窝沟封闭剂的完全保存率高于对照组,差异有统计学意义(P均<0.05)。观察组龋均值水平低于对照组,差异有统计学意义(P<0.05)。两组患儿治疗期间不良事件总发生率结果相近,差异无统计学意义(P>0.05)。结论窝沟封闭与氟化物涂膜联合应用具有更好的预防性治疗效果,可降低患龋率以及邻面龋的发生率,改善窝沟封闭剂的保存情况,儿童龋均值水平更低。
Objective To explore the application effect of pit and fissure sealant combined with fluoride coating in pre⁃venting dental caries of children's first permanent molars.Methods A total of 120 children who underwent oral physi⁃cal examination at Tai'an Stomatological Hospital from August 2021 to July 2022 were randomly selected as the re⁃search objects and divided into two groups according to different treatment methods,with 60 cases in each group.The control group only used pit and fissure sealant to treat the deep pits and fissures on the occlusal surface,while the ob⁃servation group added fluoride coating for preventive treatment on the basis of the control group.The preservation sta⁃tus of sealant,caries incidence mean of caries rate mean of caries and the occurrence of adverse events at 6 months,12 months and 18 months after treatment were compared between the two groups.Results At 18 months after sealing,the caries rate of the observation group was 1.61%(1/62),which was significantly lower than 14.89%(7/47)of the con⁃trol group,and the difference was statistically significant(χ^(2)=5.118,P<0.05).At 12 months and 18 months after seal⁃ing,the complete preservation rate of pit and fissure sealant in the observation group was higher than that in the con⁃trol group,and the differences were statistically significant(all P<0.05).In the observation group,the mean level of car⁃ies were lower than that in the control group,the difference was statificant significant(P<0.05).The total incidence rates of adverse events during treatment were similar between the two groups of children,and the difference was not statistically significant(P>0.05).Conclusion The combined application of pit and fissure sealant and fluoride coating has better preventive treatment effect,which can reduce the caries incidence rate and the incidence rate of proximal surface caries,improve the preservation status of pit and fissure sealant,and lower the mean caries level of children.
