微创AO钛制弹性髓内钉治疗儿童长骨干骨折的疗效分析

摘要 目的分析儿童长骨干骨折采用微创AO钛制弹性髓内钉治疗的效果。方法选择2020年2月—2023年10月黎平县人民医院收治的70例长骨干骨折患儿为研究对象,根据手术方法的不同分为两组,各35例。对照组采取切开复位钢板螺钉内固定,观察组采取微创AO钛制弹性髓内钉内固定。比较两组术中指标(切口长度、术中出血量、手术时间)、术后指标(住院时间、开始负重时间、骨折愈合时间)、血液流变学指标(红细胞聚集指数、纤维蛋白原、血浆黏度、全血低切黏度、全血高切黏度)、手术疗效、并发症发生情况。结果与对照组相比,观察组切口长度更小,手术时间更短,出血量更少,差异有统计学意义(P均<0.05)。观察组术后住院时间、开始负重时间、骨折愈合时间均短于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组血液流变学指标均低于对照组,差异有统计学意义(P均<0.05)。观察组治疗总有效率为94.29%(33/35),高于对照组的77.14%(27/35),差异有统计学意义(χ^(2)=4.200,P=0.040)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论儿童长骨干骨折采取微创AO钛制弹性髓内钉内固定治疗可以改善围术期指标,促进骨折良性愈合。 Objective To analyze the effect of minimally invasive AO titanium elastic intramedullary nail in the treatment of long bone fracture in children.Methods A total of 70 children with long bone fracture admitted to Liping County People's Hospital from February 2020 to October 2023 were selected as the research objects,and were divided into two groups according to different surgical methods,with 35 cases each.The control group was treated with open reduction and plate screw internal fixation,and the observation group was treated with minimally invasive AO titanium elastic intramedullary nail fixation.The intraoperative indexes(incision length,intraoperative blood loss,operation time),postoperative indexes(hospitalization time,starting weightbearing time,fracture healing time),hemorheological indexes(erythrocyte aggregation index,fibrinogen,plasma viscosity,whole blood low shear viscosity,whole blood high shear viscosity),surgical efficacy and complications were compared between the two groups.Results Compared with the control group,the incision length of the observation group was smaller,the operation time was shorter,and the amount of bleeding was less,the differences were statistically significant(all P<0.05).The postoperative hospitalization time,starting weight-bearing time and fracture healing time in the observation group were shorter than those in the control group,and the differences were statistically significant(all P<0.05).The hemorheological indexes of the observation group were lawer than those of the control group after treatment,and the differences were statistically significant(all P<0.05).The total effective rate of treatment in the observation group was 94.29%(33/35),which was higher than 77.14%(27/35)in the control group,and the difference was statistically significant(χ^(2)=4.200,P=0.040).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Minimally invasive AO titanium elastic intramedullary nail internal fixation for children with long bone shaft fractures can improve perioperative indicators and promote benign fracture healing.
