高压氧联合心脏康复训练对冠心病PCI后患者临床疗效及MACE的影响

Effects of hyperbaric oxygen therapy combined with cardiac rehabilitation training on clinical outcomes and MACE in patients with coronary heart disease after PCI

摘要 目的探讨高压氧联合心脏康复训练对冠心病经皮冠状动脉介入术(PCI)术后患者临床疗效及主要不良心血管事件(MACE)的影响。方法选取2021年8月至2022年8月济宁医学院附属医院心内科择期行PCI冠心病患者112例作为研究对象。按照随机数字表法将112例患者分为对照组(n=56)和观察组(n=56)。对照组患者采用常规心脏康复训练,观察组在对照组治疗的基础上给予HBO治疗。采用视觉模拟评分(VAS)评估治疗前后2组患者疼痛程度,并采用VAS评估临床治疗疗效。采用酶联免疫吸附法和比浊法检测2组患者治疗前后血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、降钙素原(PCT)、血清肌钙蛋白I(cTnI)、肌酸激酶(CK)、乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)水平。记录2组患者治疗后1年内MACE发生情况。结果治疗后观察组VAS明显低于对照组(P<0.01);观察组患者治疗总有效率(96.43%)明显高于对照组(83.93%),差异有统计学意义(χ^(2)=3.940,P=0.046)。治疗后观察组血清IL-6、TNF-α、CRP、PCT、cTnI、LDH、CK、CK-MB水平明显低于对照组,差异有统计学意义(P<0.05或P<0.01)。观察组MACE总发生率(7.14%)明显低于对照组(23.24%),差异有统计学意义(P<0.05)。结论高压氧联合心脏康复训练治疗冠心病PCI后,可明显降低患者血清炎症因子及心肌酶谱水平,能显著改善患者心功能,降低术后MACE的发生率,具有较好的临床应用价值。 Objective To investigate the effects of hyperbaric oxygen therapy combined with cardiac rehabilitation training on clinical outcomes and major adverse cardiac events(MACE)in patients with coronary heart disease after percutaneous coronary intervention(PCI).Methods A total of 112 patients with coronary heart disease who underwent elective PCI in the Department of Cardiology at the Affiliated Hospital of Jining Medical University from August 2021 to August 2022 were selected as the study subjects and randomly divided into control group(n=56)and observation group(n=56)using the random number table method.The control group received conventional cardiac rehabilitation training,and the observation group received hyperbaric oxygen therapy on the basis of the treatment in the control group.The visual analogue scale(VAS)was used to assess the pain levels of patients in both groups before and after treatment,and the clinical efficacy was also evaluated using VAS scores.Enzyme-linked immunosorbent assay(ELISA)and turbidimetric methods were used to detect serum levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),procalcitonin(PCT),serum troponin I(cTnI),creatine kinase(CK),lactate dehydrogenase(LDH),and creatine kinase-MB(CK-MB)in both groups before and after treatment.The incidence of major adverse cardiovascular events(MACE)within one year after treatment in both groups was recorded.Results After treatment,the VAS scores in the observation group were significantly lower than those in the control group(P<0.01).The total effective rate of the observation group(96.43%)was significantly better than that of the control group(83.93%)(χ^(2)=3.940,P=0.046).After treatment,the serum levels of IL-6,TNF-α,CRP,PCT,cTnI,LDH,CK,and CK-MB in the observation group were significantly lower than those of the control group(P<0.05 or P<0.01).The total incidence of MACE in the observation group(7.14%)was significantly lower than that in the control group(23.24%)(P<0.05).Conclusion Hyperbaric oxygen therapy combined with rehabilitation training after PCI can effectively reduce serum inflammatory factors and myocardial enzyme levels,markedly improve the cardiac function,and decrease the probability of postoperative MACE,demonstrating good clinical application value.
