摘要
目的探讨钙粒蛋白A(S100A8)和钙粒蛋白B(S100A9)在多发性骨髓瘤(MM)患者骨髓组织中的表达情况及临床意义。方法回顾性选取2017年12月至2019年4月在宜宾市第一人民医院收治的MM患者93例作为MM组。选取同期骨髓功能正常的非血液系统疾病患者45例作为对照组。采用实时荧光定量PCR(qRT-PCR)法检测MM组及对照组骨髓组织中S100A8、S100A9 mRNA表达水平,免疫组织化学染色检测S100A8、S100A9蛋白表达水平。比较两组骨髓组织中S100A8、S100A9 mRNA和蛋白表达情况。分析S100A8、S100A9表达与临床病理特征的关系;Kaplan-Meier生存分析S100A8、S100A9与MM患者生存率的关系;Pearson分析S100A8、S100A9表达水平相关性;多因素Cox回归分析影响MM患者预后因素。结果MM骨髓组织中S100A8、S100A9 mRNA表达水平分别为1.95±0.44、2.74±0.52,均高于对照组(1.04±0.23、0.98±0.20),S100A8、S100A9蛋白阳性率分别为62.37%、72.04%,均高于对照组(22.22%、26.67%),差异均有统计学意义(P<0.05)。S100A8、S100A9表达与ISS分期、β2-MG和LDH水平有关(P<0.05)。S100A8和S100A9阳性表达患者3年累积生存率分别为34.10%和35.72%,均显著低于二者阴性表达者(63.10%、62.00%),差异均有统计学意义(P<0.05)。MM骨髓组织中S100A8、S100A9表达水平呈正相关(r=0.682,P<0.05)。β2-MG、S100A8和S100A91是MM患者预后危险因素(P<0.05)。结论MM骨髓组织中S100A8、S100A9高表达,与MM病理特征和患者预后有关,可能作为MM患者潜在的预后标志物。
Objective To investigate the expression and clinical significance of calgranulin A(S100A8)and calgranulin B(S100A9)in bone marrow of patients with multiple myeloma(MM).Methods A total of 93 patients with MM admitted to the First People's Hospital of Yibin from December 2017 to April 2019 were retrospectively selected as the MM group.Forty-five patients with non-hematological diseases with normal bone marrow function were selected as the control group.qRT-PCR method was used to detect S100A8 and S100A9 mRNA levels in bone marrow tissues of 93 MM patients and 45 controls,the protein expression levels of S100A8 and S100A9 were detected by immunohistochemistry.The mRNA and protein expressions of S100A8 and S100A9 in bone marrow tissues were compared between the two groups.The relationship between S100A8 and S100A9 levels and clinicopathological features was analyzed.The relationship between the expression of S100A8 and S100A9 and the survival rate of MM patients was analyzed using Kaplan-Meier survival analysis.The correlation between expression levels of S100A8 and S100A9 was analyzed using Pearson analysis.The prognostic factors of MM patients were analyzed by multivariate Cox regression analysis.Results The mRNA expression levels of S100A8 and S100A9 in MM bone marrow tissues were 1.95±0.44 and 2.74±0.52,respectively,which were higher than those in the control group(1.04±0.23 and 0.98±0.20),the positive rates of S100A8 and S100A9 protein were 62.37%and 72.04%,respectively.which were higher than those in the control group(22.22%and 26.67%),the differences were statistically significant(P<0.05).The expressions of S100A8 and S100A9 were correlated with ISS stage,β2-MG and LDH levels(P<0.05).The 3-year cumulative survival rates of patients with positive expression of S100A8 and S100A9 were 34.10%and 35.72%,respectively,which were significantly lower than those with negative expression of both(63.10%,62.00%),and the differences were statistically significant(P<0.05).The expression levels of S100A8 and S100A9 were positively correlated(r=0.682,P<0.05).β2-MG,S100A8 and S100A91 were prognostic risk factors for MM patients(P<0.05).Conclusion S100A8 and S100A9 are highly expressed in bone marrow of MM patients.They are related to the pathological characteristics and survival of MM,and may be potential prognostic markers for MM patients.
