摘要
目的:比较电动鼻腔冲洗仪与一次性鼻腔冲洗球两种鼻腔冲洗方法对鼻咽癌患者生活质量及鼻黏膜放射损伤的影响。方法:选取2015年9月至2016年6月在广西医科大学第四附属医院接受根治性放射治疗的60例鼻咽癌患者,随机分为研究组和对照组,每组30例。研究组在放射治疗期间采用气水式鼻腔洗器进行鼻咽冲洗护理,对照组使用一次性鼻腔冲洗球进行鼻咽冲洗护理,两组患者的冲洗液均为蒸馏水。采用鼻腔鼻窦结局测试-20量表(SNOT-20中文版)评估患者的生活质量,并通过鼻咽镜观察患者放射治疗后鼻黏膜放射损伤情况。结果:在放射治疗前及放射治疗早期,两组SNOT-20评分比较差异无统计学意义(P>0.05);与对照组比较,研究组放射治疗中期及放射治疗后期的SNOT-20评分明显降低,放疗后鼻咽黏膜损伤程度明显降低,差异均有统计学意义(均P<0.05)。结论:放射治疗期间使用气水式鼻腔冲洗器进行鼻咽冲洗护理,可减轻鼻咽癌患者鼻黏膜反应,改善患者放疗中期、后期的生活质量。
Objective:To compare the influence of two methods of nasal irrigation on the quality of life and nasopharyngeal mucosal reactions of patients with nasopharyngeal carcinoma(NPC).Methods:From September 2015 to June 2016,60 patients with NPC receiving radical radiotherapy in our hospital were selected,and randomly divided into a study group and a control group,with 30 cases in each group.The nasopharynx flushing nursing was carried out with air-water nasal lavage device during radiotherapy in the study group,and the nasopharyngeal irrigation was conducted by single-use nasal irrigator in the control group.The quality of life was evaluated by Sino-Nasal Outcome Test(SNOT-20 Chinese version).The nasal mucosa damage after radiotherapy was assessed.Results:There was no significant difference in SNOT-20 score between the two groups before and at the early stage of radiotherapy(P>0.05).Compared with the control group,the SNOT-20 score of the study group was significantly decreased in the middle and late stages of radiotherapy,and the degree of nasopharyngeal mucosal injury was markedly attenuated in the study group after radiotherapy(P<0.05).Conclusion:The application of air-water nasal irrigation device during radiotherapy could alleviate nasopharyngeal mucosal reactions of NPC patients,and improve the quality of life in middle-late stage of radiotherapy.
