摘要
目的 探讨生脉注射液及生脉与丹参注射液合用对阻塞性黄疸术后炎症介质及肾功能的影响。方法 将阻塞性黄疸与无黄疸的肝胆外科患者分成 4组 ,每组 15例。黄疸对照组及无黄疸对照组术后给予一般治疗 ,生脉注射液 (SM) ,生脉与丹参注射液 (SS)合用治疗阻塞性黄疸组 ,术后 1～ 6d分别给予生脉及生脉与丹参合用。动态观察血浆内毒素 (LPS)、肿瘤坏死因子 (TNF -α)、白细胞介素 - 6、白细胞介素 - 8(IL - 6 ,IL - 8)、内皮素 (ET)以及尿转铁蛋白 (UTFR)、尿白蛋白 (UALB)和尿视黄醇 (URBP)。结果 生脉注射液及生脉与丹参注射液合用治疗组术后 7d与术后 1d相比 ,LPS、IL - 6、IL - 8、ET、TNF -α、UTFR、URBP、UALB明显下降 (P <0 .0 5 ) ,且低于或接近术前水平 ,其中UALB ,URBP与黄疸对照组术后 7d相比也有显著性差异 (P <0 .0 5 ) ;另外 ,生脉与丹参合用组术后 7d与生脉组相比ET ,TNF -α ,IL - 6 ,URBP有显著性差异 (P <0 .0 5 )。结论 生脉及生脉与丹参合用对阻塞性黄疸患者术后血浆LPS有拮抗作用 ,同时降低其它炎症介质水平 ,进而有保护肾脏功能的作用 ;
OBJECTIVE To investigate the effect of Shengmai injection(SM), or combining with Compound dansheng injection and Shengmai injection(SS) on inflammatory mediators and renal function in patients with obstructive jaundice(OJ).METHODS This study included four groups: the OJ patients receiving SS for 6 days after operation(n=15); the OJ patients receiving SM for 6 days after operations; the OJ patients not receiving SS and SM(n=15);the patients with other hepato-biliary disorders(Non-OJ, n=15).The levels of ET, LPs, TNF-а, IL-6, IL-8 and urine RBP, TFR, ALB were measured one day before operations and 1, 4, 7 days after operations. RESULTS In the SM and SS groups,LPs, TNF-а, IL-6, IL-8, ET, URBP, UTFR and UALB levels were obviously lower on the 7 th after operation compared with those on the 1 st after operation(P<0.05). The urine RBP and ALB levels were significantly lower in the SS group than in the OJ group(P<0.05). On the other hand, In the SS group, ET, TNF-α, IL-6, URBP levels were obviously lower on the 7 th day after operation compared with the SM group. CONCLUSION The post-operative use of salvia miltiorrhiza and Shengmai for patients with obstructive jaundice may effectively decrease the postoperative levels of plasma LPs, ET, TNF-α, IL-6 and IL-8. At the same time,Compound dansheng injection and Shengmai may protect renal function by inhibiting inflammatory mediators and improving blood dynamics.
