摘要
目的研究大肠息肉临床病理与CD24、胸苷激酶(TK1)表达的关系。方法选取我院消化内科2014年1月至2015年9月期间收治的88例大肠息肉患者,其中炎性息肉、管状腺瘤、管状绒毛状腺瘤、绒毛状腺瘤各22例,同时选取同期住院的正常大肠黏膜和腺癌各22例作为对照组;观察CD24、TK1的表达水平与息肉组织类型、息肉异型增生、息肉大小及息肉数量的关系。结果 CD24、TK1水平的表达随着组织类型的递增而逐渐增高(P<0.05),正常黏膜、炎性息肉、管状腺瘤、管状绒毛状腺瘤、绒毛状腺瘤、腺癌CD24水平阳性率分别为0(0/22)、0(0/22)、27.3%(6/22)、40.9%(9/22)、45.5%(10/22)、90.9%(20/22),TK1水平阳性率分别为0(0/22)、0(0/22)、9.1%(2/22)、18.2%(4/22)、27.3%(6/22)、50.0%(11/22);随着异型增生程度的增加,CD24、TK1水平的阳性表达也逐渐增加(P<0.05),无增生、轻度增生、中度增生、重度增生CD24水平阳性率分别为0(2/22)、50.0%(11/22)、72.7%(16/22)、90.9%(20/22),TK1水平阳性率分别为0(2/22)、45.5%(10/22)、59.1%(13/22)、90.9%(16/22);随着息肉直径的增加,CD24、TK1水平的阳性表达率逐渐增加,且呈现相关性(P<0.05),直径≤1.0 cm、1.1~1.9 cm、≥2.0 cm CD24水平阳性率分别为18.2%(4/22)、50.0%(11/22)、81.8%(18/22),TK1水平阳性率分别为22.7%(5/22)、45.5%(10/22)、90.9%(16/22);单发、多发CD24水平阳性率分别为4.5%(1/22)、68.2%(15/22),TK1水平阳性率分别为9.1%(2/22)、63.6%(14/22),差异均有统计学意义(P<0.05)。结论 CD24、TK1水平在大肠息肉中呈高表达,而增生的程度不一、大小不一、数量不同、病理类型不同的腺瘤,其表达的程度也不同。
Objective To study the correlation of serum CD24 and TK1 expression levels with clinicopathological features in patients with colorectal polyps. Methods A total of 88 patients with colorectal polyps(including inflammation polyp, tubular adenoma, tubular villous adenoma, and villous adenoma, with 22 cases in each type), who admitted to Department of Gastroenterology of our hospital from January 2014 to September 2015, were selected as the research objects. At the same time, 22 cases of normal colorectal mucosa and 22 cases of adenocarcinoma were chosen as the control group. The correlation of serum CD24 and TK1 expression levels with types of polyps, polyp dysplasia, polyp size and number of polyps were observed and compared. Results The expression levels of CD24 and TK1 increased gradually with tissue type(P<0.05); CD24 positive rates, in normal mucosa, inflammatory polyp, tubular villous adenoma,villous adenoma, adenoma were respectively 0(0/22), 0(0/22), 27.3%(6/22), 40.9%(9/22), 45.5%(10/22), 90.9%(20/22), and TK1 positive rates in normal mucosa, inflammatory polyp, tubular villous adenoma, villous adenoma,adenoma were respectively 0(0/22), 0(0/22), 9.1%(2/22), 18.2%(4/22), 27.3%(6/22), 50.0%(11/22). With the increase of the degree of dysplasia, the positive expression levels of CD24 and TK1 gradually increased(P<0.05); CD24 positive rates in non-hyperplasia, mild hyperplasia, moderate hyperplasia and severe hyperplasia were respectively 0(2/22), 50.0%(11/22), 72.7%(16/22), 90.9%(20/22), and TK1 positive rates in these were respectively 0(2/22), 45.5%(10/22), 59.1%(13/22), 90.9%(16/22). With the increase of the diameter of polyps, the positive expression levels rate of CD24 and TK1 increased gradually(P<0.05); CD24 level positive rate of <1.0 cm in diameter, 1.1~1.9 cm, ≥2.0 cm were respectively 18.2%(4/22), 50.0%(11/22), 81.8%(18/22), and TK1 level positive rates of these diameter were respectively22.7%(5/22), 45.5%(10/22), 90.9%(16/22). There was significant difference between the single polyps(4.5%(1/22)vs 9.1%(2/22)) and polyposis(68.2%(15/22) vs 63.6%(14/22)) in the level positive rate of CD24 and TK1(P<0.05).Conclusion The expression levels of TK1 and CD24 were high in colorectal polyps, but with the differences in the degree of hyperplasia, pathological types, number and size of the tumors, the expression levels were different.
