摘要
目的采用meta分析综合定量评价低氧环境对人体睡眠的影响,为高海拔低氧环境下睡眠研究提供理论基础。方法以'低氧睡眠'、'高海拔睡眠'、'高原睡眠'、'altitude and sleep'、'hypobaric hypoxia and sleep'、'normal hypoxia and sleep'、'polysomnography and altitude'等为检索词检索各数据库,并建立纳入和排除标准,采用Revman 5.3软件对所有纳入文献的结局指标进行统计分析。结果共纳入研究文献13篇,研究对象共210名成年人,其中男性168名,女性42名,每名受试者均进行了常氧和低氧两个环境下的睡眠监控。敏感性分析表明研究结果较稳定,漏斗图分析提示不存在明显的发表偏倚。meta分析结果显示,低氧环境对机体睡眠过程中的睡眠效率(合并效应值为-2.79,95%CI:-4.59^-1.00,P<0.01),慢波睡眠比例(合并效应值为-4.47,95%CI:-6.38^-2.57,P<0.01),血氧饱和度(合并效应值为-12.40,95%CI:-16.91^-7.89,P<0.01)均有明显影响,而对快速动眼期睡眠比例无明显影响(合并效应值为-1.99,95%CI:-5.94^-1.97,P>0.05)。结论低氧环境会导致成年人睡眠障碍,表现为有效睡眠时长缩短、睡眠深度比例降低。
Objective To use the meta-analysis method to comprehensively and quantitatively evaluate the impact of hypoxic environment on human sleep,and provide theoretical basis and topic reference for the study of sleep in high altitude and low oxygen environment.Methods The terms,such as hypoxia sleep,high altitude sleep,plateau sleep,altitude and sleep,hypobaric hypoxia and sleep,normal hypoxia and sleep,polysomnography and altitude,were searched in each related database and the inclusion and exclusion criteria was established.The Revman5.3 software was used to statistically analyze all outcomes included in the literature.Results This study included 13 research papers.The study involved 210 adult subjects,including 168 males and 42 females.In the study,each subject underwent sleep monitoring in both normoxia and hypoxia settings.Sensitivity analysis showed that the results of this study were relatively stable.Further analysis of the funnel plot suggested that there was no significant publication bias in this study,indicating that the results were highly reliable.Meta-analysis showed that the hypoxic environment had a significant effect on sleep efficiency during sleep(WMD=-2.79,95%CI:-4.59--1.00,P<0.01),and slow wave sleep ratio(WMD=-4.47,95%CI:-6.38--2.57,P<0.01),blood oxygen saturation(WMD=-12.40,95%CI:-16.91--7.89,P<0.01),while no significant effect on REM sleep ratio was seen(WMD=-1.99,95%CI:-5.94--1.97,P=0.32).Conclusion The hypoxic environment can cause sleep disorders in adults,which is characterized by a shortened effective sleep duration and a decrease in the proportion of deep sleep.
