摘要
目的观察含益生菌的肠内营养对重型颅脑损伤大鼠胃溃疡指数(UI)、胃黏膜血流量(GMBF)的影响。方法取54只SD大鼠建立重型颅脑损伤致急性胃黏膜损伤动物模型,再随机分为对照组、肠内营养组(EN组)和生态营养组(EC组)各18只。EC组用灌喂针向大鼠胃内灌入三九全营素加双歧杆菌三联活菌胶囊,营养液热量为1673.6kJ/100g,喂养量为598.19kJ/(kg·d),每天灌喂3次。EN组用灌喂针向大鼠胃内灌入三九全营素,热量及喂养方法、频次同EC组。对照组用同样喂养方法灌喂等剂量的生理盐水,每天3次。喂养后第1、3、5天取标本,采用激光多普勒血流仪测定GMBF,计算UI。每组每个时相点选择6只大鼠完成测量。结果EN组喂养第3、5天UI与对照组比较显著下降,GMBF显著升高(P<0.05,P<0.01)。EC组与对照组相比3个时相点UI显著下降,GMBF显著升高(均P<0.01)。EC组与EN组比较,喂养第3、5天UI显著下降,GMBF显著增加(均P<0.05)。结论大鼠重型颅脑损伤后给予含益生菌的肠内营养可以增加胃黏膜血流量,降低胃黏膜损伤指数,对重型颅脑损伤大鼠的胃黏膜有一定的保护作用。
Objective To observe the effect of enteral nutrition added with probiotic bacteria on ulcer index(UI), and gastric mucosal blood flow(GMBF) in rats with severe brain injury. Methods A total of 54 rats were used to build up the model of severe brain injury, and were randomly and equally divided into 3 groups: a control group, an enteral nutrition group (group EN) and an econutrition group(group EC). Group EC were fed with Trensine and Bifid Triple Viable Capsules by using a feeding needle, 3 times a day, with the calories of the mixture being 1 673.6 kJ/100 g and total calories of 3 feedings being 598.19 kJ/(kg ·d). Group EN were fed with Trensine by using a feeding needle directly 3 times a day, and the calories needed were the same as group EC. The control group were fed with equal volume of normal saline. On the 1st, 3rd, 5th day of feeding, GMBF was measured by using Laser Doppler Flowmeter and UI was calculated. Each group sacrificed 6 rats on each tested day. Results UI of group EN on the 3rd and 5th day of feeding were significantly lower than that of the control group, and GMBF were significantly higher(P<0.05,P<0.01). UI of group EC on the 3 tested days were significantly lower than that of the control group, and GMBF were significantly higher(P<0.01 for all). Compared with group EN, UI of group EC on the 3rd and 5th day of feeding were significantly lower, and GMBF were significantly higher(P<0.05 for all). Conclusion Enteral nutrition added with probiotic bacteria for severe brain injury rats can increase GMBF and decrease UI, which means enteral nutrition added with probiotic bacteria may protect severe brain injury rats from gastric mucosal lesion induced by stress.
