摘要
目的:观察缺血性脑血管病患者的MRA诊断情况与DSA结果的对比研究及脑血管狭窄分布情况。方法:选择100例在住院期间同时进行过MRA和DSA检查的缺血性脑血管病患者,分别记录患者的MRA及DSA结果并进行统计学分析。结果:100例患者MRA检查共发现81例患者173条血管病变,DSA检查共发现86例患者118条血管病变,而MRA未发现病变但DSA诊断出狭窄病变13例(13%)。各血管MRA结果与DSA结果进行χ2检验,其中ACA、MCA、PCA三条血管结果有显著性差异(P<0.05),其余血管无显著性差异。所有血管DSA结果统计发现轻度狭窄10例,中度狭窄39例,重度狭窄40例,闭塞25例,发育不良4例。结论:绝大多数缺血性脑血管病患者均存在不同程度的脑血管狭窄病变,MRA检查对于颅外血管及后循环血管狭窄的判断基本与DSA一致,但在大脑前动脉、大脑中动脉、大脑后动脉的血管成像与DSA结果有显著性差异,DSA仍然是判断脑血管狭窄的金标准。
Objective To observe the comparison of MRA diagnosis and DSA results and cerebrovascular stenosis distribution of patients with ischemic cerebrovascular disease. Methods 100 parients with ischemic cere-broascular disease who had the examinatinations by both MRA and DSA at the same time in hospital were select-ed, and MRA and DSA results of the patients were recorded respectively and were analyzed statistically. Results Of 100 patients, there were 81 cases of patients with 173 vascular lesions found by MRA examination, and 86 patients with 118 vascular lesions found by DSA examination, and there were 13 cases (13%) of stenosis lesions detected by DSA but undetected by MRA. MRA and DSA results of each vessel were conducted χ2 test, of which, the results of there vessels including anterior cerebral artery(ACA), middle cerebral artery(MCA) and pos-terior cerebral artery (PCA) had significant differences, while the rest vessel had no significant difference. Of all vascular DSA results, there were 10 cases with mild stenosis, 39 cases with dysplasia. Conclusion The vast ma-jority of patients with ischemic cerebrovascular disease had different levels of cerebrovascular stenosis, and diag-nosis of extrac cranial vessels and posterior ciculation vascular stenosis by MRA is consistent with DSA, but MRA and DSA results have significant differences in the ACA, MCA, PCA imaging. DSA is still 'dold standard"for diagnosis cerebrovascular stenosis.
