摘要
目的 :探究内镜配合胃蛋白酶原(PG)、胃泌素-17(G-17)检测对早期胃癌的诊断价值。方法 :将2014年2月~2017年1月我院收治的74例浅表性胃炎患者、55例萎缩性胃炎患者和51例早期胃癌患者作为研究对象。采用ELISA法测定三组患者的血清PGⅠ、PGⅡ及G-17水平,计算PGR值。利用ROC曲线分析PGⅠ、G-17单独或联合检查在诊断早期胃癌上的临床价值,并于手术前后观察胃癌患者的各项指标变化。结果 :早期胃癌组的PGⅠ、PGR水平明显低于浅表性胃炎组和萎缩性胃炎组,其G-17水平则明显高于浅表性胃炎组和萎缩性胃炎组,差异比较有统计学意义。ROC分析结果显示,血清PGⅠ、G-17的临界值分别为73μg/L和4pmol/L。PGⅠ、G-17单独或联合诊断的线下面积分别为0.831、0.687、0.885,敏感度和特异度分别为82.35%、64.71%、86.27%和72.55%、60.78%、80.39%。联合诊断的线下面积、敏感度及特异度均显著高于PGⅠ、G-17单独诊断。结论 :早期胃癌患者的PGⅠ、PGR水平明显下降,其G-17水平异常升高。PG、G-17联合检测配合内镜检查有助于提升早期胃癌的检出率,对胃癌的早期筛查具有重要意义。
Objective To research the diagnostic value of endoscopy combined with pepsinogen(PG) and gastrin-17(G-17) in early gastric cancer. Method From February 2014 to January 2017, 74 patients with superficial gastritis, 55 patients with atrophic gastritis and 51 patients with early gastric cancer in our hospital were selected as the research objects. Measured the serum levels of PGⅠ, PGⅡand G-17 in three groups by ELISA, and calculated the PGR value. Observed the indexes of gastric cancer patients before and after operation. Result The levels of PGI and PGR in the early gastric cancer group were significantly lower than those in the superficial gastritis group and the atrophic gastritis group, and the level of G-17 was significantly higher than that in superficial gastritis group and the atrophic gastritis group, and the differences were statistically significant. ROC analysis showed that the critical values of serum PGI and G-17 were 73 g/L and 4 pmol/L, respectively. The area under curve of PG I or G-17 alone or in combination were 0.831, 0.687 and 0.885, respectively, and the sensitivity and specificity were 82.35%, 64.71%, 86.27% and 72.55%, 60.78%, 80.39%, respectively. The area under curve, sensitivity and specificity of combined diagnosis were significantly higher than those of PG I and G-17 respectively. Conclusion The levels of PG, I and PGR in patients with early gastric cancer were significantly decreased, and the G-17 level is abnormally elevated. PG and G-17 combined with endoscopy is helpful to improve the detection rate of early gastric cancer, and this is important for early screening of gastric cancer.
