摘要
目的 :比较电化学发光法(ECL)、快速血浆反应素环状卡片试验(RPR)、梅毒螺旋体明胶颗粒凝集试验(TPPA)及胶体金法四种方法检测梅毒的灵敏度及特异性,为临床上梅毒的血清学筛查提供有效的方法。方法 :收集2017年1月~2017年12月份我院确诊的梅毒患者60例作为观察组,同时选择同期体检的健康体检者60例作为对照组。采用ECL、RPR、TPPA、胶体金法检测两组血清样本,比较四种方法的灵敏度、特异性、准确率、阳性预测值、阴性预测值。使用卡方检验对数据进行统计学分析。结果 :ECL、TPPA阳性检出率显著高于胶体金法,RPR阳性检出率高于胶体金法,TPPA阴性检出率高于其他三种检测方法,差异均不具有统计学意义。TPPA和ECL的灵敏度和阴性预测值高于胶体金法,TPPA和ECL准确率高于RPR法。TPPA灵敏度、特异度、准确率、阴性预测值、阳性预测值高于ECL,差异均不具有统计学意义。结论 :ECL和TPPA法灵敏度、准确率和特异度均较好,可先使用ECL和RPR法进行双筛,再经TPPA进行确诊。
Objective To compare the sensitivity and specificity of electrochemiluminescence(ECL), rapid plasma response card ring assay(RPR), treponema pallidum gelatin agglutination test(TPPA) and colloidal gold method for the detection of syphilis. And to provide effective methods for clinical screening of syphilis serology. Methods We collected 60 cases of syphilis diagnosed in our hospital from January 2017 to December 2017 as the observation group. And 60 healthy physical examination persons at the same period were taken as control group. The serum samples in two groups were detected by ECL, RPR, TPPA and colloidal gold. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of the four methods were compared. The data were statistically analyzed with chi square test. Result The positive rate of ECL and TPPA was significantly higher than that of colloidal gold method. The positive rate of RPR was higher than that of colloidal gold method, but difference was not statistically significant. The sensitivity and negative predictive value of TPPA and ECL were higher than that of colloidal gold method, and the accuracy of TPPA and ECL was higher than that of RPR. The sensitivity, specificity, accuracy, negative predictive value, and positive predictive value of TPPA were higher than those of ECL, but the difference was not statistically significant. Conclusion ECL and TPPA methods have good sensitivity, accuracy and specificity. It can be double-screened with ECL and RPR, and then diagnosed by TPPA methods.
