摘要
目的:以北京地区4所三甲医院为例,调查中医住院医师规范化培训中医临床思维培养现状。方法:采用电子版本自填式问卷,从“对中医临床思维培养重要性的认知”“目前规培中中医临床思维培养存在问题”“影响规培中培养中医临床思维的原因”“规培中如何培养中医临床思维能力”4个方面对在北京地区4所三甲医院参与规培的研究生进行调查。结果:共103名参与者接受了本调查,88份问卷纳入统计分析。超过80.00%的受访者认同“中医临床思维是保持和提高中医药临床疗效优势的基础,是中医药传承发展的核心内容,中医临床思维培养应作为规培的重要目标之一”。超过60.00%的受访者认为规培中中医临床思维培养“西化”“弱化”和“僵化”问题突出,其原因包括整体社会环境的影响以及当前中医规培管理评价机制尚未能充分突出中医临床思维。超过50.00%的受访者认为将传统师承模式引入规培,能有效培养中医临床思维。其外,门诊抄方、参与教学查房、总结带教老师临床经验,也是提高中医临床思维能力的重要途径。结论:中医临床思维作为中医药传承发展的核心内容,如何培养中医临床思维已成为规培的重要目标之一。面对规培中中医临床思维培养“西化”“弱化”和“僵化”问题突出的现状,在规培中强调跟诊式教学、案例教学和实践教学意义尤为重大。
Objective:Taking 4 grade ⅢA hospitals in Beijing as an example,the current situation of TCM clinical thinking training in the standardized training of Chinese medicine resident doctors was investigated.Methods:Using the electronic version of self-administered questionnaires,the graduate students who participated in the regulation of 4 grade ⅢA hospitals in Beijing were investigated from four aspects:(1)cognition of the importance of TCM clinical thinking training;(2)current problems in the training of TCM clinical thinking in the regulation of training;(3)influencing the reasons for training TCM clinical thinking in the regulation of training;(4)how to cultivate TCM clinical thinking ability in regular training".Results:A total of 103 participants accepted the survey and 88 questionnaires were included in the statistical analysis.More than 80.00%of respondents agree that"Chinese medicine clinical thinking is the basis for maintaining and improving the clinical efficacy of traditional Chinese medicine.It is the core content of the inheritance and development of Chinese medicine.The cultivation of TCM clinical thinking should be one of the important goals of regulation and training".More than 60.00%of the participants believed that the problems of"westernization","weakening"and"rigidity"in the training of TCM clinical thinking were prominent.The reasons included the impact of the overall social environment and the current evaluation mechanism of TCM clinical thinking management had not fully highlighted.More than 50.00%of the participants believed that introducing the traditional teacher-to-student model into regular training could effectively cultivate the clinical thinking of TCM.Besides,it is also an important way to improve the clinical thinking ability of TCM to copy prescriptions,participate in teaching rounds and summarize the clinical experience of teachers.Conclusion:Clinical thinking of traditional Chinese medicine(TCM)is the core content of TCM inheritance and development.How to cultivate clinical thinking of TCM has become one of the important objectives of training.In the face of the current situation of the"westernization","weakening"and"rigidity"of TCM clinical thinking training in the cultivation of Chinese medicine,it is particularly important to emphasize follow-up teaching,case teaching and practical teaching in the TCM training.
