摘要
目的:探讨介入联合中医辨证治疗股骨头缺血性坏死的临床疗效。方法:2009年1月-2011年1月选取股骨头缺血性坏死FicatⅠ-Ⅲ患者50例,随机分为单纯介入治疗组22例(36髋)和联合治疗组28例(44髋)。单纯介入治疗组采用Seldinger穿刺技术注入罂粟碱30 mg、尿激酶6万单位、低分子右旋糖酐100 mL、复方丹参注射液30 mL治疗;联合治疗组在介入治疗的同时给予中医辨证治疗。所有患者均在治疗前和治疗6个月后、12个月后行DSA、CT和MRI检查,并对患髋进行Harris评分及股骨头静脉造影评级,比较治疗前后的变化。结果:治疗后的第6个月、第12个月,两组Harris评分较治疗前均明显提高,且联合治疗组优于单纯介入治疗组(P<0.05)。两组间股骨头静脉造影评级治疗后较治疗前均有明显提高(P<0.05),且联合治疗组提高更明显。结论:介入联合中医辨证治疗股骨头缺血性坏死疗效显著,可改善股骨头血供、促进修复、改善临床症状。
Objective: To discuss the clinical effects of the treatment of avascular necrosis of femoral head( ANFH) by intervention treatment combined with TCM differentiation. Methods: 50 cases of FicatⅠ- Ⅲ with ANFH got from January,2009 to January, 2011 were randomly divided into the pure intervention group( 22 cases,36 pelvic bones) and the treatment group( 28 cases,44 pelvic bones). For the pure intervention group,papaverine of 30mg,urokinase of 60 thousand units,low molecular dextran of 100ml,compound danshen injection of 30ml were injected by Seldinger percutaneous technique. The combination treatment group were given oral Chinese decoction plus the intervention treatment. The DSA,CT and MRI of all the patients were examined before the treatment,6 months after the treatment,and 12 months after the treatment. The pelvic bones were graded by Harris and the level of the femoral vein angiography was detected. The changes before and after the treatment were observed. Results: 6 and 12 months after the treatment,the Harris scores of both groups were greatly improved by comparing that before the treatment,and the combination treatment group were better than the pure intervention group( P < 0. 05). The level of the femoral vein angiography of both groups after the treatment were greatly enhanced by comparing that before the treatment( P < 0. 05),and the improvement of the combination treatment group was significant. Conclusion: TCM differentiation combined with intervention treatment,has remarkable therapeutic effects for the ANFH,and improves the blood supply of the femur head,promotes the recovery and improves the clinical symptoms.
