葡聚糖微球经微导管栓塞术中应用参数的实验研究 被引量:9

摘要 目的 :探讨葡聚糖微球经不同微导管栓塞治疗时的应用参数 ,减少血管栓塞术中微导管的阻塞。方法 :( 1)光镜及电镜下观测葡聚糖微球 ,并测定葡聚糖微球栓子悬液的悬浮稳定性。( 2 )用注射器人工推注和高压注射器自动注射 ,测试 5种不同内径的微导管注射 2种不同规格 ( 5 0~ 15 0μm,10 0~ 30 0μm )各 3种不同浓度 ( 2 5 mg/ ml,5 0 m g/ml,10 0 mg/ ml)的葡聚糖微球 ,记录注射时使用的压力和导管阻塞情况。 ( 3)应用 SPSS9.0软件对结果作统计学处理。结果 :( 1)葡聚糖微球原液加入等量的造影剂悬浮稳定性好 ;( 2 )不同规格的注射器注射相同的葡聚糖微球产生的推注压力不同 ;( 3)经不同的微导管注射相同的葡聚糖微球时推注压力不同。用内径通过≤ 0 .0 14英寸导丝的微导管注射葡聚糖微球将会堵管。结论 :提倡介入栓塞术中使用 5 ml的注射器透视下推注栓子 ,避免使用内径配≤ 0 .0 Objective:To evaluate applicable coefficients of the optimal Dextan microspheres that may be injected reliably through microcatheters during therapeutic embolization.Methods:The properties and characteristics of Dextran Microspheres in vitro were studied.Five different microcatheters were tested.Two different size ranges of Dextran microspheres at three different concentrations were tested with each catheter.For each size and concentration of Dextran microspheres,sixty 2ml injections were made into each catheter tested by injecting through syringes and automatic hyperpressure injector.Both objective and subjective assessments were recorded.Statistical analysis was performed by SPSS9.0.Results:We found that when diluted in an equal mixture of contrast material and saline,the particles remained in stabe suspension.We also found the recommended size ranges and concentrations of Dextran Microspheres injected with reasonable pressure and minimal risk of inadvertent catheter occlusion.Both the large and small particles of Dextran microspheres could not be injected through catheters used with ≤0.014 inch guide wires.Conclusion:Dextran Microspheres are good solid embolic agents,for they remain in suspension long,don't clump or shatter.Reasonable Dextran microspheres size ranges and concentrations that can be injected through large microcatheters(≥0.014 inch guide wire) will help prevent inadvertent catheter occlusion during vascular embolization.5ml syringe is recommended to use during the clinical interventional procedures.
