摘要
目的探讨口服营养补充对住院老年患者临床结局的影响。方法收集2016年1月-2017年6月期间在河北省人民医院老年科就诊的患者258例,随机分为治疗组和对照组,两组患者均根据实际需要营养值给予相应的饮食指导。治疗组在膳食的基础上增加口服营养补充,对照组在膳食的基础上增加传统膳食补充。两组均连续使用四个疗程,7 d为一疗程。入院时及治疗第7天、第14天、第21天、第28天测量两组患者的反映体脂、骨骼肌总量的指标,实验室检查营养指标水平变化及统计临床结局,并进行统计分析。结果经治疗后,两组体质量、三头皮褶厚度、上臂围、上臂肌围、FM、FFM等均有不同程度升高(P<0.05)。实验组治疗后体重、三头皮褶厚度、上臂围、上臂肌围、FM、FFM均显著高于对照组(P<0.05),体脂比率显著低于对照组(P<0.05)。两组血清白蛋白、前白蛋白、血红蛋白、转铁蛋白等水平均逐渐升高(P<0.05)。实验组治疗后血清白蛋白、前白蛋白、血红蛋白、转铁蛋白水平显著高于对照组(P<0.05)。两组患者住院时间无明显差异(P>0.05),实验组患者住院费用、治疗药占比低于对照组,差异具有统计学意义(P<0.05)。结论进行合理、规范的口服营养补充后老年患者营养状况得到较明显的改善,减少患者经济费用,提高患者疗效,为临床中制定规范化临床营养治疗体系提供理论依据。
Objective To explore the effect of oral nutrition supplementation on clinical outcomes of elderly inpatients.Methods 258 cases of geriatric patients in Hebei General Hospital from January 2016 to June 2017 were randomly divided into two groups.And both groups were given dietary guidance according to their requirements.The experimental group was supplemented with oral nutrition on the basis of diet,while the control group was supplemented with traditional diet on the basis of diet.The two groups were treated for four consecutive courses,seven days as a course of treatment.We obtained the indexes reflecting body fat and skeletal muscle mass,the changes of nutritional indexes and clinical outcomes at admission and on the 7th,14th,21st and 28th day of treatment and statistical analysis.Results After treatment,the body weight,the thickness of triceps fold,the circumference of upper arm,the circumference of upper arm muscle,FM and FFM of the two groups increased in different degrees(P<0.05).The weight,thickness of triceps fold,arm circumference,muscle circumference,FM and FFM of the experimental group were significantly higher than those of the control group(P<0.05),and the body fat ratio was significantly lower than that of the control group(P<0.05).The levels of serum albumin,prealbumin,hemoglobin and transferrin in both groups increased gradually(P<0.05).After treatment,the levels of serum albumin,prealbumin,hemoglobin and transferrin in the experimental group were significantly higher than those in the control group(P<0.05).There was no significant difference in hospitalization time between the two groups(P>0.05).The hospitalization expenses and the proportion of therapeutic drugs in the experimental group were lower than those in the control group,with statistical significance(P<0.05).Conclusion After rational and standardized oral nutrition supplementation,the nutritional status of elderly patients has been significantly improved,the economic cost of patients has been reduced,and the curative effect of patients has been improved,which provides a theoretical basis for the formulation of standardized clinical nutrition treatment system in clinic.
