摘要
目的 了解新生儿感染性肺炎的不同病原对免疫功能的影响。 方法 采用免疫酶法( EL ISA)检测肺炎患儿血中腺病毒 3型 ( ADV3)、腺病毒 7型 ( ADV7)、埃可病毒 ( ECHO)、巨细胞病毒 ( CMV)、柯萨奇 B病毒 ( Cox B)、副流感病毒 ( PIV)、呼吸道合胞病毒 ( RSV)和单纯疱疹病毒 1型 ( HSV1 ) 8类常见病毒及支原体 ( MP)特异性 Ig M和 T细胞亚群 ( CD3、 CD4 、 CD8和 CD4 /CD8) ,免疫球蛋白 ( Ig G、 Ig A和 Ig M)及 C反应蛋白 ( CRP) ,以 CRP≥ 0 .0 2 g/ L作为诊断细菌性感染的界限值。将肺炎组患儿分为 4组。 结果 肺炎组 111份血清中 4 0份阳性 ,对照组 30份均阴性 ;8型常见病毒及 MP特异性 Ig M阳性 5 0例次 ,其中 ADV315 .3% ( 17/ 111) ,ECHO 9.0 %( 10 / 111) ,CMV 6 .3% ( 7/ 111) ,Cox B和 MP各 4 .5 % ( 5 / 111) ,PIV 3.6 % ( 4 / 111) ,RSV和 ADV70 .9% ( 1/ 111) ,HSV1 未检出 ;CD3和 CD4 细胞数肺炎组少于对照组 ;Ig A和 Ig M含量肺炎组高于对照组 ( P<0 .0 5 ) ;CD3和 CD4 细胞数及 Ig M含量细菌性肺炎组与对照组两两比较 ,Ig A含量病毒及支原体肺炎组分别与其它肺炎组和对照组两两比较 ,差异有显著性 ( P<0 .0 5 ) ;CD8、CD4 / CD8、Ig G各组比较 ,差异无显著性 ( P>0 .0 5 )。 结论?
Objective To explore immune status of the newborn of infectious pneumonia with different pathogen. Methods Special serum IgM of 8 types viruses (ADV 3,ECHO,CMV, CoxB ,PIV,RSV,ADV 7,HSV 1) and mycoplasma (MP) and T lymphocyte subpopulation (CD 3、CD 4、CD 8 and CD 4/CD 8),immunoglobulin (IgG、IgA and IgM) were detected in infants with infectious pneumonia and control infants. Results Special serum IgM of 8 types viruses and MP were positive in 40 cases (36 0 %) of 111 infants with pneumonia (ADV 3,ECHO,CMV,CoxB,MP,PIV,RSV and ADV 7 were 5 3 %,9 0 %,6 3 %,4 5 %,4 5 %,3 6 %,0 9 % and 0 9 % respectively).All were negative in 30 control infants.The number of CD 3 and CD 4 cells in bacterial pneumonia group were significantly lower than the control group.Serum IgA and IgM concentration in pneumonia group were higher than those in control group.Serum CD 3、CD 4 and IgM concentration in bacterial pneumonia group were higher than those in control groups (P<0 05).Serum IgA concentration in viral and mycoplasma pneumonia groups were higher than that in other pathogenic pneumonia group and control groups(P<0 05).There were no significant difference between various types pneumonia groups and control group on the number of CD 8 cell,CD 4/CD 8,serum IgG concentration Conclusion viruses and mycoplasma are important pathogens on neonatal pneumonia in this locality.The number of CD 3 and CD 4 cell in newborns with infectious pneumonia are suppressed by bacterial infection,but not by viral and mycoplasma infection.Serum IgA can be increased by viral and mycoplasma infection,and serum IgM can be increased by bacterial infection.The number of CD 8 cell,CD 4/CD 8,serum IgG were not changed in viral and bacterial infection.
