摘要
目的 研究先天性巨细胞病毒 ( cylomegalovirus,CMV)感染患儿外周血白细胞介素- 2 ( interleukin 2 ,IL - 2 )、γ干扰素 ( Interferon,IFNγ)水平的变化与 CMV病毒负荷的相关关系。 方法 用荧光定量聚合酶链反应 ( fluorogenic quantitative polym erase chain reaction,FQ - PCR)定量测定外周血 CMV- DNA ,31例足月新生儿 CMV- DNA阳性为感染组 ,并同时检测 CMV- Ig M。选取同期非感染新生儿 (外周血 CMV- DNA及抗 CMV- Ig M均阴性 ) 2 9例为对照组。 2组患儿应用EL ISA法检测 IL - 2及 IFNγ水平 ,并于 4周后复查。 结果 感染组患儿外周血 CMV- DNA定量中位数为 2 .75× 10 4 copy/μl,IL - 2和 IFNγ水平明显降低 ,与对照组比较 ,差异有显著性 ( P<0 .0 1) ;IL - 2、 IFNγ水平与 CMV- DNA基因定量值呈负相关 ,r分别为 - 0 .7882、 - 0 .6 75 1( P<0 .0 1) ;4周后复查 30例患儿 CMV- DNA转阴 ,其 IL - 2、 INγ水平上升 ,与 CMV- DNA阳性时比较 ,差异有显著性 ( P<0 .0 1) ,与对照组比较 ,差异无显著性 ( P>0 .0 5 ) 结论 先天性 CMV感染时机体外周血 IL - 2、 IFNγ水平明显降低 。
Objective To study the influence of peripheral blood virus load of congenital CMV infection to type 1 T helper. Methods Peripheral blood CMV DNA were detected quantitatively by fluorogenic quantitative polymerase chain reaction(FQ PCR) in the full term infants in our NICU,31 infants with positive result as infection group,and CMV IgM were detected at the same time.29 non infected infants with negative results were the control group.Interleakin 2(IL 2)、Interferon(IFN γ) were detected by ELISA in all infants.After 4 weeks,CMV DNA、IL 2、IFN γ and CMV IgM were detected again in infection group ;IL 2、IFN γ were also detected again in control group. Results The median of CMV virus load in peripheral blood of infection group was 2 75×10 4 copy /μl. The levels of IL 2 and IFN γ of infection group were significantly lower than those of control group(P<0 01).The levels of IL 2、IFN γ correlated negatively with CMV virus load in peripheral blood(r=-0 7882,-0 6751,P<0 01).CMV was disappear in peripheral blood in 30 infants after 4 weeks ,and the levels of IL 2、IFN γ increased significantly compared with the original levels (P<0 01). Conclusion The levels of IL 2,IFN γ in peripheral blood in congenital CMV infected infants decreased significantly.The function of cell immunity mediated by CTL is depressed and the degree relate significantly to virus load in peripheral blood.
