大型听神经鞘瘤与脑干、小脑关系的分型初探:55例报告 被引量:5

摘要 目的 探讨大型听神经鞘瘤与脑干、小脑的位置关系对手术方法和治疗效果的影响。方法 根据影像资料及手术录像 ,将我科近 6年经显微手术治疗的 5 5例大型听神经鞘瘤分为I型 (脑干腹侧型 )、Ⅱ型 (脑干小脑型 )和Ⅲ型 (嵌入型 ) ,统计临床表现、手术方法和预后 ,进行回顾性分析。结果 本组肿瘤全切率为 91% ;面神经解剖保留率为 80 % ;听神经解剖保留率为 2 2 % ;本组无死亡。嵌入型肿瘤手术难度最大。结论 熟悉桥小脑区相关静脉的解剖 ,根据肿瘤与脑桥、小脑的位置关系分型 ,采取相应的手术策略 ,有助于提高手术效果。 Objective To analyze the relationship between brain stem and large acoustic neuroma and cerebellum in microsurgery. Methods 55 patients with large acoustic neuroma in our department during the last 6 years were divided into three groups on the basis of radiological features and operation record. The clinical features, microsurgical technique and results were analyzed retrospectively. Results Total tumor removal was accomplished in 91% of the patients. Anatomical preservation of the facial nerve and the cochlear nerve were achieved in 80% and 22%, respectively. There was no death in this group. The resection of type III tumor was the most difficult. Conclusions Surgical results of large acoustic neuroma can be improved by familiarity with the anatomy of CPA and adoption of corresponding surgical strategy according to its relationship to brain stem and cerebellum.
