摘要 目的 探讨基因 p16在胃癌中表达的临床意义特别是与胃癌预后的关系。 方法 用ABC法检测本院 1991年 1月至 1998年 12月间经手术切除的 15 2例胃癌的p16表达状况 ,分析 p16的表达与胃癌分期、组织学类型、分化程度、病灶部位、肿瘤大小及生存率的关系。用Cox比例风险模型评估影响预后的因素。结果 在 15 2例胃癌中 ,p16的表达率为 72 4% ,p16阴性及阳性胃癌在 5年生存率、年龄、性别、肿瘤大小、肿瘤分化程度等方面有显著性差异 ,肿瘤TNM分期及p16的表达状况是影响胃癌预后的独立因素。结论 p16的表达状况可作为判断胃癌预后的指标。 Objective To investigate the clinical significance of p16 expression in gastric carcinoma with special reference to the prognosis. Methods One hundred and fifty-two gastric carcinomas undergoing operation in our hospital between 1991 and 1998 were evaluated for expression of the protein expression of p16 utilizing Avidin-Biotin immunohistochemistry techniques. Statistical correlations with stage, histologic type, differentiation degree, location, size, and overall survival were analysed. The Cox proportional hazard model was also performed to evaluate which factors had an independent prognostic value. Results Of 152 resected gastric cancers, 110(72.4%) were p16 positive. Differences were observed in the frequency of p16 positivity with respect to 5-year survival rate, age, gender, tumor size, and histological differentiation degree.In a multivariate analysis, TNM stage and the expression of p16 were independent prognostic factors in gastric cancer. Conclusion The results of the current study suggest that expression of p16 may be a useful prognostic factor for patients with gastric carcinoma.
