轻度认知障碍患者疾病进展相关的脑功能连接度特征分析 被引量:4

Feature Analysis of Brain Functional Connectivity Degree Regarding Disease Progression in Patients with Mild Cognitive Impairment

摘要 目的基于磁共振脑功能连接度分析,探讨健康受试者(CN)、稳定状态轻度认知障碍(SMCI)与进展的轻度认知障碍(PMCI)患者基线期全脑功能连接度差异,并评估有差异的脑区对MCI向AD转化的预测价值。方法研究中所有被试者统计信息、MMSE评分、3D-T1WI数据信息均来自ADNI。对数据进行预处理并行全脑长程、短程功能连接(FC)度值计算,分别采用SPSS及SPM8中的单向ANOVA设计,定量分析人口统计数据和全脑长程、短程FC度值的差异,然后提取PMCI与CN组之间具有显著FC差异的脑区的平均度值,计算相应的AUC值。结果(1)基于体素的统计分析揭示了FC度存在显著组间差异的特征性脑区;(2)颞下回的FC度值在预测MCI向AD进展方面有显著的AUC。结论 CN组、SMCI组与PMCI组存在显著的脑功能连接模式差异,且颞下回的功能连接的逐步下降对预测MCI向AD进展可能具有一定的提示意义。 Objective To investigate the difference in functional connectivity degree across the whole-brain among the three groups of healthy subjects(CN),stable mild cognitive impairment(SMCI),and progressive mild cognitive impairment(PMCI)based on resting-state f MRI data,and assess the prognostic value of the identified connectivity difference for conversion from MCI to AD.Methods Demographic information、MMSE score and f MRI data of all of the subjects in this study were obtained from ADNI.The preprocessed data were used to calculate the long-range and short-range functional connectivity(FC)degree maps of the whole-brain.The differences between the demographic data and whole-brain longrange and short-range FC degrees were analyzed by One-way ANOVA in SPSS and SPM8 respectively.Then the mean degree value of the brain area with significant FC difference between the PMCI and CN groups was extracted and the corresponding AUC value was calculated.Results(1)Voxel-based statistical analysis revealed several brain regions with significant differences in functional connectivity among the three groups;(2)FC degree in the right inferior temporal gyrus was found with significant AUC value for the prognosis of disease progression.Conclusion There are significantly different brain connectivity patterns among CNs,SMCIs,and PMCIs,and the gradual decline of functional connectivity in inferior temporal gyrus may have a certain significance in predicting the progression of MCI to AD.
