摘要
目的:研究双切口金属植入物内固定修复配合中药伤骨再生汤治疗Schatzker Ⅴ、Ⅵ型胫骨平台双髁骨折临床疗效。方法:选择2017年2月-2018年2月本院接诊的96例SchatzkerⅤ、Ⅵ型胫骨平台双髁骨折患者按抽签法分为研究组、对照组,各48例。对照组施行双切口金属植入物内固定修复治疗,研究组在此基础上服用伤骨再生汤治疗。比较两组疗效、Rasmussen放射学与膝关节功能评分、骨转换生化标志物及并发症。结果:研究组患者的临床疗效91. 67%(44/48)高于对照组的72. 92%(35/48),差异有统计学意义(P <0. 05);两组Rasmussen放射学评分及膝关节评分高于对照组,差异有统计学意义(P <0. 05);两组CTX、PINP水平均低于治疗前,研究组CTX、PINP相比对照组更低,差异有统计学意义(P <0. 05);研究组患者术后并发症发生比例2. 08%(1/48)低于对照组的16. 67%(8/48),差异有统计学意义(P <0. 05)。结论:双切口金属植入物内固定修复疗法配合伤骨再生汤可有效降低骨转换生化标志物水平及炎症因子含量,促进骨折愈合,安全性高。
Objective:To evaluate the efficacy of the Shanggu Zaisheng Decoction combined with double-incision metal implant internal fixation in the treatment of Schatzker V and VI tibial plateau fractures.Methods:From February 2017 to February2018,96 cases of patients with Schatzker V and VI tibial plateau fractures were enrolled in the study.All the patients were divided into study group and control group by weighting method,with 48 cases in each group.The patients in the control group were given double-incision metal implant internal fixation for treatment,and those in the study group were treated with Shanggu Zaisheng Decoction on the basis of the control group.The efficacy of the two groups,Rasmussen radiology and knee function score,bone turnover biochemical markers[type I collagen cross-linked carboxy terminal peptide(CTX),serum type I procollagen amino terminal propeptide(PINP)]and complications were compared.Results:The clinical efficacy of the study group was 91.67%(44/48),higher than that of the control group(72.92%(35/48)(P<0.05).After treatment,the Rasmussen radiology and knee function scores of the two groups were higher than those before treatment,and the Rasmussen radiology and knee function scores of the study group were higher than those of the control group(P<0.05).The levels of CTX and PINP in the two groups were lower than those before treatment.The CTX and PINP of the study group were lower than those of the control group(P<0.05).The proportion of postoperative complications in the study group was 2.08%(1/48),which was lower than that in the control group[16.67%(8/48)](P<0.05).Conclusion:Double-incision metal implant internal fixation combined with Shanggu Zaisheng Decoction is effective in treating Schatzker V and type tibial plateau fractures,which can effectively reduce bone turnover biochemical markers and inflammatory factors,promote fracture healing and have high safety.
