摘要
目的探讨永久性乙状结肠造口的手术细节及其并发症的防治要点。方法总结1997～1999年间152例行永久性乙状结肠造口的直肠肛管癌患者的造口手术操作要点和近期并发症的发生情况,分析并发症发生的原因及防治对策。结果所有患者的造口手术均按统一规范进行,早期并发症发生率较低,主要有水肿(7.9%,12/152)、缺血(1.3%,2/152)和延迟排气(2.6%,4/152)。无严重并发症出现。结论规范的手术可使永久性乙状结肠造口手术的并发症发生率明显降低,其要点是合适的术前定位、精心细致的手术操作、良好的血液供应和防止感染。
Objective To analyze surgical procedures of permanent sigmoid colostomy and essential points to prevent stomal c omplications.Methods From 1996to 1999,152cases with rectoanal cancer received permanent sigmoid c olostomy in our department.Surgical procedures,reasons and preventio n for short-term stomal complication s were analyzed.Results The colostomy procedures in all case s were performed according to uniform stan dards.The occurrence rate of short-term stomal complications was low.The complications mainly included e dema (7.9%,12/152),ischemia(1.3%,2/152)and delayed exsufflation from stoma (2.6%,4/152).There existed no severe stomal comp lications.Conclusion Standardized surgical procedures,particularly preoperative locatio n,skillful procedures,good blood s upply to stoma and prevention of postopera tive infection are the keys to preven t short-term stomal complications o f permanent sigmoid colostomy.[
